Building trust with people experiencing homelessness often takes months of consistent, in-person contact before someone is ready to accept care.

In our Street Medicine partnership with UC Davis Health, our team cultivates trusting relationships with patients experiencing homelessness that keep returning for the same unmanaged condition. Our CHWs keep clients connected to physicians like Dr. Satterwhite out on the street or in shelter, long after they’ve left the hospital.

Access to care is about showing up consistently. Community HealthWorks is proud to be an organization that never stops showing up for our community members and that keeps people experiencing homelessness connected to care.
