FHT, FHO...did you know each of these acronyms has something to do with primary healthcare?

We are the FHT (Family Health Team). This is a model of care found throughout Ontario. Here in Peterborough, our team supports family doctors working in an FHO (Family Health Organization).

Healthcare providers working at PFHT (Peterborough Family Health Team) provide patients with one-on-one support, such as seeing their registered dietitian or mental health clinician. We are also helping unattached patients (meaning those without a family doctor) through our Connect Clinic!
