As our community continues recovering from the recent wildfires, our friends at Coordinated Care and community partners are offering two opportunities for support this week:

Service Update: Beginning Monday, August 17, primary care appointments with Excelsior Family Medicine will return to in-person visits at 3754 W. Indian Trail Road.

All other services will remain virtual or off-site at this time. We will continue to provide updates as additional services return to the Indian Trail location.

We’ve mobilized!
Because Excelsior Family Medicine cannot currently provide in-person medical care at our Indian Trail location, Molina Healthcare has mobilized its mobile health clinic to help us continue serving patients and neighbors affected by the Old Trails Fire.

Priority will be given to patients who already had appointments scheduled with Excelsior Family Medicine. Those patients will be contacted directly.

If you do not have an appointment but need medical care, or would like to connect with a Community Health Worker, please call us. Excelsior Family Medicine serves our community regardless of insurance status or ability to pay.
