Our Vision: Our vision is to create a dementia friendly community where older adults and their caregivers can feel supported and have access to the services they need to successfully navigate the challenges of aging while maintaining quality of life. A description of the services will include:
I. Provide funds to support the direct care needs of financially qualified individuals
• Assessment of
needs – A written evaluation and assessment of needs identified between client and certified care manager
• Care Coordination – the coordination of care services in the community to help meet identified needs, this may include equipment or devices to support safety and enable independence
• Placement Assistance – Identify resources, level or care and placement options for those who are not able to remain safely in the home
• Caregiver support through credentialed caregivers who provide private duty caregiving services in the home – clearly define criteria caregiver must meet
• Provide equipment, devices, and therapeutic aids as deemed beneficial in the written assessment
II. Provide Community Education and Awareness on Dementia and Other Disabilities
• Fund local education activities
• Coordinate recognition events with other parties
• Administer a caregiver mentor program
• Promote, support and fund a Dementia Friendly Town initiative
• Offer information on the benefits of music and work with others on implementing the “Music & Memory” initiative
