🚨NEW PROGRAM ALERT🚨

The EMBRACE+ Whole-Person Navigation Program is a community-centered initiative designed to support individuals disproportionately impacted by HIV by improving access to prevention, care, and essential services. Through culturally responsive peer navigation, participants receive personalized guidance to access HIV testing, treatment, and prevention options, including PrEP.

The program takes a holistic approach by addressing the broader factors that influence health, offering support with housing stability, employment readiness, mental health and substance use services, and enrollment in medical care and benefits.

This will be done through community outreach, education, and ongoing 1:1 support.
