This September, we're celebrating the partnerships that help older adults stay healthy, connected, and independent. Today, we're proud to spotlight Meals on Wheels of Boulder!

Every day, our partnership makes a difference for the people we serve. Whether participants receive meals at the TRU PACE Day Center or enjoy home-delivered meals, TRU PACE and Meals on Wheels of Boulder work together to provide support that helps older adults continue living independently and with a higher quality of life.

Kim and Lehshel from Meals on Wheels of Boulder hear incredible stories from participants about the impact of these services. Clients share how meals and supportive connections have improved their health, brought them joy, increased their comfort, and helped them feel safer and more cared for.

One participant, Susan, has received home-delivered meals for almost 10 years. She often says the meals have "saved her life" and doesn't know what she would do without them. Today, thanks to the partnership between Meals on Wheels of Boulder and TRU PACE, she receives even more support to help her remain independent and thriving.

Thank you, Meals on Wheels of Boulder, for helping us nourish lives, build connections, and support older adults throughout our community.

Therapy can support the life someone wants to keep living.

At TRU PACE, physical, occupational, and speech therapy are part of the all-inclusive care model and may be recommended to help participants stay active, confident, and supported in daily life.

Physical therapy can support strength, balance, walking, and steadiness.

Occupational therapy can support daily tasks like dressing, bathing, cooking, and moving safely around the home.

Speech therapy can support communication, safe swallowing, memory strategies, and ways to make talking and understanding easier.

Each therapy plan is personalized and added to a participant’s care plan when the participant and care team agree it can support their health and independence.

With TRU PACE, therapy is connected to the full care team, helping participants receive support that reflects their goals, their abilities, and the home life they want to maintain.

Understanding TRU PACE starts with a conversation.

Before applying, an enrollment specialist can help older adults and families understand what TRU PACE is, how the program works, and what eligibility may look like.

TRU PACE provides care for older adults who want to continue living safely in their community while maintaining independence. The program can support participants at home and in the community through a managed medical plan designed to help meet their healthcare needs.

An enrollment specialist can help explain:

1. Whether someone meets the basic age requirement of 55 or older
2. Whether they live in the TRU PACE service area
3. How nursing facility level of care is determined by the state
4. What it means to live safely in the community with TRU PACE support
5. How care works through TRU PACE contracted providers

Taking the first step can feel more manageable when you know what questions to ask.

A strong caregiving plan gives everyone a clear place to start.

Caregiving often includes many moving parts: appointments, medications, transportation, meals, home routines, social connection, and changing health needs. When responsibilities are shared, a simple plan can help families stay organized and confident.

Helpful steps include:

1. Write down the most important care needs
2. Match each task with a specific person
3. Create a shared calendar or checklist
4. Keep notes from appointments in one place
5. Make time to talk through updates together

Eligible older adults at TRU PACE receive coordinated support through an interdisciplinary care team and a personalized plan of care designed around their needs and goals.

A steady routine can make the day feel more manageable.

For older adults, daily routines can bring comfort, confidence, and a stronger sense of stability. Familiar rhythms like regular meals, movement, medication reminders, social time, rest, and meaningful activities can help support physical, emotional, and social well-being.

A daily routine may help you:

1. Feel more grounded throughout the day
2. Keep meals, medications, and appointments on track
3. Build in time for movement and rest
4. Stay connected through regular social moments
5. Create a sense of purpose and independence

TRU PACE supports whole-person care by helping eligible older adults build support around their health, routines, goals, and daily life.

Older adults have spent a lifetime learning, solving problems, caring for others, building traditions, and gathering stories that deserve to be shared.

One meaningful way to support connection and purpose is to create space for that knowledge to keep moving through the community.

You can celebrate the wisdom older adults bring by:

1. Asking about a skill they learned through work, family, or life experience
2. Inviting them to share a favorite recipe, tradition, song, or story
3. Encouraging them to teach a hobby, craft, game, or practical skill
4. Listening to their perspective when making family or community decisions
5. Helping them stay connected to groups, activities, and conversations where their voice matters

At TRU PACE, whole-person care includes supporting connection, purpose, and meaningful participation in daily life. Through coordinated care, supportive services, and social engagement, eligible older adults can continue sharing who they are with the communities they love.
