Rooted in the geographic center of New York State, in the Crown City of Cortland, NY and surrounding rural areas, FHN works to enhance the health and well being of individuals from underserved, rural communities by providing comprehensive, affordable medical and dental services. These services are delivered in a caring, professional and family friendly environment, that is sensitive to the diverse health care needs and challenges of our patients, while encouraging the development of strong healthy lives. Cortland County is a designated Health Professional Shortage Area (HPSA) for the low-income population. This designation denotes areas having shortages of primary care, dental care, or mental health providers based upon statistics of the community. FHN’s current HPSA designation for medical, dental, and mental health are respectively: 20, 24, and 22. FHN has 5 freestanding Health Centers, one which includes dental, and an additional 4 School-Based Health Centers:

Our School-Based Health Centers are located in Appleby Elementary School, Marathon Jr/Sr High School, DeRuyter Central School, and the Cincinnatus Central School. If you need to reach a Family Health Network Provider for a non-life threatening issue when the Health Centers are closed, please call 607-756-0405 or toll free at 1-833-954-0489. One of our Professional Staff is on-call 24 hours a day, 365 days a year. If it is an emergency, please call 9-1-1.

*This Health Center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.

🎉 BIG NEWS FOR CENTRAL NEW YORK! 🎉

🏥 Family Health Network of Central New York, Inc. (FHN) is proud to announce the launch of a new Family Medicine Residency Training Program in partnership with Larkin Health System of Miami, Florida!

📅 Beginning July 1, 2026, FHN will welcome its first resident physicians as part of an ACGME-accredited Rural Track Program designed to train the next generation of family physicians in community-based, full-spectrum primary care.

🌟 Why This Matters

For FHN, a Federally Qualified Health Center (FQHC) that has served Cortland and contiguous counties since 1972, the program represents a long-term investment in the region's health. Rural communities across the country face persistent shortages of primary care physicians, and residents who train in a community often choose to stay and practice there.

👩‍⚕️👨‍⚕️ At full enrollment, the program will train four family medicine residents at FHN, helping strengthen healthcare access for local families while preparing physicians to serve rural and medically underserved communities.

At FHN, the residents will train under the guidance of Dr. Douglas Rahner, who will oversee their clinical education at the Central New York site. The program is directed by Dr. Ivan Rodriguez, Program Director of the family medicine residency at Larkin in Miami.

👏 Meet Our First Residents!

FHN is proud to welcome the program's two inaugural residents, both of whom bring extensive clinical experience to their family medicine training:

🌍 Hina Askri, MD grew up in Lahore, Pakistan, and earned her medical degree from Fatima Jinnah Medical University. She brings approximately 15 years of healthcare experience, including a four-year postgraduate fellowship in Obstetrics and Gynecology and clinical work caring for underserved patients in New York and New Jersey. She is drawn to outpatient primary care for the opportunity to build lasting relationships with patients and help them stay healthy through prevention and continuity of care.

🌍 Thayane Lemes, MD is originally from Rio de Janeiro, Brazil, where she earned her medical degree from Faculdade Souza Marques and completed dual residencies in Obstetrics and Gynecology. A permanent U.S. resident now based in Florida, she brings years of clinical experience, including work in reproductive health, and is fluent in English, Spanish, and Portuguese, an asset for serving FHN's diverse patient community. As a family medicine resident, Dr. Lemes approaches patient care by listening closely to her patients' needs, respecting their values, and building relationships grounded in trust. What she loves most about her work are the strong relationships she forms with the people she cares for.

As the residents complete their training in Central New York, they will provide care to FHN patients and families across the health center's service area, deepening the organization's capacity to meet community needs while shaping the next generation of rural family physicians.

🌟Meet the Provider – Coming Soon to Family Health Network!🌟

Family Health Network is excited to welcome Hina Askri, MD to our team! As part of FHN's new residency training partnership with Larkin Health System of Miami, FL, Dr. Askri is joining FHN as a family medicine resident physician — bringing her remarkable breadth of clinical experience to our patients and families while training under the guidance of Dr. Douglas Rahner. Dr. Askri will begin seeing patients on July 2, 2026 at the Cortland Family Practice location.

Dr. Askri grew up in Lahore, Pakistan, a city that in her words is vibrant and known for its rich culture, history, and strong sense of community. She brings approximately 15 years of healthcare experience to FHN and the communities we serve. She earned her medical degree from Fatima Jinnah Medical University in Lahore and went on to complete a four-year postgraduate fellowship in Obstetrics and Gynecology at Sir Ganga Ram Hospital, where she managed high-risk pregnancies, performed complex surgical procedures, and mentored junior medical staff. Hina is no stranger to our state. Before joining the family medicine residency at Larkin Community Hospital, she gained additional clinical experience caring for underserved populations at Essen Health Care in the Bronx, New York.

Dr. Askri's path into medicine was shaped by a desire to make a difference. Growing up, she saw healthcare as a profession through which she could help others, and she was drawn to a career where she could “make a meaningful difference in people's lives every day.”

Dr. Askri chose to work at a Federally Qualified Health Center (FQHC) like FHN because it allows her to care for the whole person, address healthcare disparities, and “make a meaningful impact on the health and well-being of the community.” She especially values outpatient medicine, where there is always something new to learn and where she can build long-term relationships with her patients. As she puts it, “it is especially rewarding to help patients manage their health proactively and prevent conditions from becoming serious enough to require emergency care.”

As a family medicine resident, Dr. Askri believes in a patient-centered approach. She takes the time to understand not only a patient's medical concerns but also their family, social, and personal circumstances. Her focus on “education, prevention, and continuity of care empowers patients to take an active role in their health and helps them achieve the best possible outcomes.”

Outside of work, Dr. Askri enjoys cooking and trying new recipes, shopping, and traveling with her family. She especially loves being in nature, “particularly mountains,” where she can “relax and recharge.”

Fun Fact: Dr. Askri loves exploring recipes and cuisines from different cultures and enjoys recreating dishes from her travels at home. She's also a fan of Pakistani and Turkish television dramas for their storytelling and family-centered themes, and, fittingly, medical shows like House and The Pitt.

Dr. Askri is passionate about preventive, proactive, and holistic healthcare. She believes vaccinations are one of the most effective tools we have to prevent serious illness — protecting individuals, families, and the broader community — and that preventive screenings, such as breast, cervical, and colorectal cancer screenings, help detect disease early, when treatment is often most effective. For Dr. Askri, addressing the physical, emotional, and social factors that influence health helps patients enjoy a higher quality of life and stay healthier over the long term.

Starting January 1, 2027, some people who have Medicaid coverage will need to show they are working, going to school, helping out in the community, or participating in job training to keep their health coverage. This is a new federal rule from the government called Work Requirements for Medicaid Coverage.

If this rule affects you, you’ll need to do 80 hours of approved activities each month.
