It was a pleasure to welcome Dr. Leigh Chapman, Canada’s Chief Nursing Officer, to Wellbrook Place.

During the visit, Dr. Chapman met with Candace Chartier, Interim Vice President, Long-Term Care and Quality, and Deslyn Willock, Director of Long-Term Care Operations, Quality and Risk. Together, they explored our model of care, innovative design, and the importance of supporting staff wellness.

There was a powerful sense of openness and connection at Wellbrook Place earlier this month.

South Asian older adults living with diabetes and cognitive changes, along with their caregivers, gathered to share their experiences — from the realities of daily life to the kinds of supports that would truly make a difference. Caregivers spoke about the weight they carry, and participants highlighted the importance of care that reflects their cultures, languages, and family experiences. Every story shared helped shape a clearer path forward.

This workshop is part of Aging Well with Diabetes and Dementia: Supporting Mississauga’s Immigrant Communities to Age in Place, a collaborative initiative co-led by the University of Toronto’s Leslie Dan Faculty of Pharmacy and THP’s Institute for Better Health, in partnership with Partners Community Health. We thank Kainat Bashir, Lisa McCarthy, and Kathy Lashley, along with partners deprescribing.org and the Novo Nordisk Network for Healthy Populations, for their leadership and collaboration.

Together, this work reflects a shared commitment to Connected Care and Learning — creating more inclusive, culturally responsive supports so older adults can age in place with dignity and choice. That’s what excellence looks like in action.

With daily activities, refreshments, yoga sessions, and raffles, it was an opportunity to recognize the incredible nurses who bring skill, compassion, and dedication to their work every day. Their commitment to providing high-quality care — while continuously learning and supporting one another — reflects what excellence looks like at Partners Community Health.

Happy PSW Day to every Personal Support Worker who shows up with compassion, patience, and heart. 💛

Today we're celebrating one of our own: Chelsea Haughton, a PSW at Partners Community Health who is retiring after an incredible 37 years in long-term care.

Chelsea described her workplace as her second home and her second family. Over nearly four decades, she cared for countless residents, navigated the challenges of a global pandemic, and never stopped learning.

"Working in care gives you a lot. It teaches you a lot about life."

Chelsea's story is a beautiful reflection of our May theme, Excellence: a commitment to high-quality care and a culture of continuous learning.

🥁✨ Beat. Breathe. Belong.

The Wellbrook Place East Recreation Team brought something truly special to our residents this week: DROM! Led by Tarang from TruHealth Rehab and Fitness, our certified DROM trainer, this one-of-a-kind experience blends the energy of drumming with the calm of meditation.

Residents from every floor came together in our spiritual room to drum on exercise balls, tables, and chairs, releasing stress, finding clarity, and sharing joy.

This is what excellence in care looks like. It is not just about health. It is about creating meaningful moments, nurturing the whole person, and fostering a community where everyone truly belongs. 💛

Celebrating the moms who mean everything 💐

At Wellbrook Place West, residents celebrated Mother’s Day with a heartfelt day of connection and creativity. Residents hand painted beautiful cards to share with their neighbours, and every resident, especially every mom, received a lovely carnation 🌸

With a live musical performance, the celebration was full of warmth, smiles, and meaningful moments.

Across Wellbrook Place, the day was made even more special with a visit from MPP Rudy Cuzzetto, who helped hand out carnations and celebrate the wonderful mothers in our community.

The residents truly, really enjoyed the atmosphere and the time spent together. It was a joyful and memorable occasion for all 💙

As part of our Excellence Month at PCH, we are proud to create experiences that go beyond care and help every resident feel celebrated and at home.

To all the incredible moms, grandmothers, and mother figures, today is for you. Happy Mother’s Day!
