Fostering Hope Initiative - Community Health FHI is a program of CCS that is comprised of CHWs/OHP Assisters. For more information contact: [email protected] or [email protected].

Our staff provides community health, residential, & pregnancy/parenting services in Marion, Polk, & Yamhill counties in order to help support strong families & safe, healthy neighborhoods. Fostering Hope Initiative Community Health Workers (CHWs) provide system navigation to Citizen Waived Medical and non-Citizen Waived Medical members with the focus on engaging this population through Oregon Heal

th Authority's auto-enrollment strategy in Marion and Polk counties. FHI's Community Health Workers:
· Provide one-to-one system navigation services in Marion and Polk County and assist individuals to enroll in health coverage.
· Employ strategies to overcome the barriers to utilizing Oregon Health Plan coverage and help navigate both Oregon Health Plan and other Coordinated Care Organization systems.
· Improve health literacy and navigation of health systems and health systems literacy.
· Collaborate with local community organizations and establish information sharing processes to facilitate outreach, enrollment, and system navigation.

CHW Irma helped to coordinate a wonderful Mother's Day Blessing Event at Southtown Apartments in Monmouth. This is just one example of the impact that residential services can have in our communities!

Thank you so much to Risen Life Church for assisting in the coordination of this event, including supplying materials and staffing!

We had such a fun time making flower arrangements, jewelry pieces, partaking in crafts with the kids, and showering mothers with prizes! We have a great community.
