Fostering Hope Initiative - Resident Services Many of the families who reside in affordable housing face challenges beyond stable housing.

The Fostering Hope Initiative (FHI) is a community based, collective impact initiative designed to promote the health and wellbeing of children and adults, strengthen families, and build more resilient neighborhoods. Resident services aim to support families to navigate health and human services, build community, decrease risk factors and address the social determinants of health. We are a resourc

e network for residents who are in need of supportive services, and by providing a safe environment. Service Examples:

-Assist individuals and families to identify needs, barriers and resources.

-Assistance individuals and families to develop service plans with individualized goals.

-Provide resource navigation for identified needs.

-Assistance to apply for, access and maintain benefits (CHWs are OHP Assisters).

-Assistance to locate and access physical, behavioral, and dental care services.

-Assistance to locate and access financial assistance.

-Assistance to access education regarding wellness, chronic disease management, parenting and nutrition, etc.

-Assistance to access transportation services.

-Assistance with interpretation and translation for needs identified in service plan (Spanish/English) (Chuukese/English).

-Liaison with public-school system.

-Provide COVID outreach and education.

-Provide Wrap-Supports to individuals and families in COVID quarantine and isolation.

- Fostering Hope Initiative Community Health Workers (CHWs) provide system navigation to Citizen Waived Medical and non-Citizen Waived Medical members with the focus on engaging this population through Oregon Health Authority's auto-enrollment strategy in Marion and Polk counties. Health care affordability affects people across our state in very real and personal ways.

We invited people in Oregon to share how health care costs affect them. One story came from a retired teacher in Klamath County who worries about her and her husband’s growing health care costs.

In the coming weeks, we’ll share more stories that show how health care costs affect our neighbors.
