JPNS Monthly Spotlight: This study explores the link between chronic inflammatory demyelinating polyneuropathy (CIDP) and monoclonal gammopathy of undetermined significance (MGUS). It assesses clinical phenotypes and treatment responses. https://bit.ly/4aVlDvL
IgG MGUS is significantly more common in CIDP patients, at 9%, compared to matched neurological controls at 3%. However, the prevalence of IgM and IgA paraproteins did not significantly differ between the two groups.
When comparing CIDP patients with and without IgG MGUS, those with IgG MGUS less often had an acute clinical presentation. They also more frequently experienced sensory deficits and showed prolonged distal compound muscle action potential duration
Despite these clinical differences, first-line treatment response rates were highly comparable. Eighty percent of patients with IgG MGUS responded to treatment, compared to 67 percent of CIDP patients without MGUS.
The study concludes that while IgG MGUS is associated with CIDP, its presence does not create a distinct subgroup with unique clinical features or treatment implications. First-line treatments remain equally effective for both patient groups.
