Through our activity-based therapy programs we offer the best chance at recovery, independence, and improved health. Activity-Based Therapy (ABT) is based on the research done within the NeuroRecovery Network. ABT shows that activating specific networks of the Central Nervous System leads to improvement of those specific tasks which can potentially lead to recovery. Activity-Based Therapy programs

are not covered by insurance in North Carolina. Due to the out of pocket cost to our members, typically they have used the resources allotted to them through their insurance company and are now attending our facility to maintain recovery while continuing to receive therapy. Our program falls into the continuum of care model. We are an option for individuals needing activity and recovery, long after insurance payment has been suspended and/or as an adjunct to covered therapy services. Our staff has degrees in Exercise Science, Exercise Physiology, Athletic Training and Occupational Therapy and receives regular training through the NeuroRecovery Learning Center.

Ask us about the Nuero acupuncture that Dr. Brett Hightower has been offering at NextStep Raleigh.

A question came up as to what acupuncture -specifically neuropuncture- does to help brain injury patients.

DR. Hightower's short answer is it promotes neuroplastic development.
The longer answer is that new axonal growth—primarily through axonal sprouting and compensatory rewiring—can contribute to improved function and performance after traumatic brain injury (TBI).

Key Mechanisms
In the adult central nervous system (CNS), including the brain, true long-distance axonal regeneration (regrowing severed axons over long distances to their original targets) is limited due to inhibitory factors like scar tissue, myelin-associated inhibitors (e.g., Nogo), and other molecular brakes. However, axonal sprouting—where uninjured or partially damaged neurons extend new branches or collaterals to form compensatory connections—is a well-documented part of post-TBI recovery.

• Peri-lesional sprouting: New connections form around the injury site.

• Contralateral or remote sprouting: Intact areas (e.g., opposite hemisphere) send axons to denervated regions.

• This remodeling helps restore circuits for motor, sensory, and cognitive functions.

Studies show correlations between this sprouting and better behavioral outcomes in animal models of TBI. For example, interventions enhancing sprouting (e.g., stem cells, scaffolds, growth factors, or blocking inhibitors) lead to greater functional gains in sensorimotor and cognitive tasks.

Evidence from Research
• Axonal reorganization after TBI has been linked to spontaneous and therapy-enhanced recovery.
• Therapies promoting plasticity (e.g., rehabilitation, certain drugs, or biologics) amplify this process and improve outcomes.
• In related CNS injuries (stroke, SCI), axonal sprouting causally supports motor recovery, with similar principles applying to TBI.

This week, Roc welcomed his buddies, a pair of fellow athletes, to NextStep Raleigh. Former UNC JV Basketball player Roc was joined by Donal Gooch, a 6'9" center who played for Bluefield College and the North Carolina Capitals, and Jevon Embry , a 7'0" center and power forward with professional basketball experience who has since transitioned into professional wrestling.

While the setting looked different from a basketball court or wrestling ring, the mindset was exactly the same.

At NextStep Raleigh, training leads to recovery. It is strength work, repetition, discipline, and the relentless pursuit of improvement. The goals may be different, but the competitive spirit remains unchanged.

The same determination that fueled Roc on the basketball court now drives his recovery journey, proving that an athlete's mindset extends far beyond the game.

It was a special day to see athletes from different backgrounds come together in support of one another and to witness firsthand what recovery-focused training looks like.

The same work ethic. The same commitment. The same pursuit of becoming stronger than yesterday.

As Dr. Taylor Watkins resumes her chiropractic career, we want to thank her for the energy, compassion, and positivity she brought to NextStep Raleigh every day.

Our members, staff, and community have been fortunate to enjoy her bustling around the center with her bright smile lighting up the room, making sessions a bit more enjoyable.

While we're excited for all that's ahead, she will certainly be missed around here.
