NP Family Practice is growing, and we’re looking for a compassionate, organized, and motivated Licensed Practical Nurse (LPN) – Chronic Care & Remote Patient Monitoring Coordinator to join our team! ❤️💙

This is a great opportunity for an LPN who enjoys building relationships with patients and wants to be involved in the growing field of Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and Advanced Primary Care Management (APCM).

In this role, you’ll help manage RPM patients, conduct wellness and follow-up calls, support chronic care and care coordination, provide patient education, assist with medication and treatment follow-up, and connect patients with community resources.

✨ We’re looking for someone who wants more than just a job we want someone who wants to help us improve how patients with chronic conditions are supported between office visits.
