For patients with complex chronic conditions, staying healthy at home can make all the difference.

In this Mission Moment, Jennifer Baker, PT, with St. Luke’s Home Health & Hospice, shares the story of a patient who experienced frequent hospital readmissions and faced significant challenges managing multiple chronic illnesses.

Through persistence, advocacy, and a commitment to patient-centered care, Jennifer helped ensure the patient received the support needed to remain at home, where she was happiest. Today, she is healthier, managing her conditions more effectively, and avoiding unnecessary hospitalizations.
