Our own Maureen Znoj, Senior Director of HealthForce NH, and Beth Gustafson Wheeler, FHC's Senior Director for Population Health and Equity, had the opportunity to attend AHA's Healthier Together conference which brings together leaders in health care who collaborate to improve health and address disparities in health outcomes. Speakers discussed data driven strategies and innovative approaches to advance person centered care, improve access, strengthen hospital community partnerships and promote collaborative solutions to reducing disparities in health outcomes. The conference provided the opportunity to connect with AHA team members Julie Kim, Sonia Malik, Riley Anderson and Joy Rhoden.
