We are looking for a Home Visitor to help make a positive impact on famiies working in partnership with parents/carers and other agencies to provide early education and support with a family focus, child led play and structured teaching.
Wiltshire Portage is a home-visiting education service for pre-school children with SEND and their families. Portage aims to
• work with families to help them develop a quality of life and experience, for themselves and their young children, in which they can learn together, play together, participate and be included in their community in their own right.
• play a part in minimising the disabling barriers that confront young children and their families.
• support the national and local development of inclusive services for children.

We are looking for a Home Visitor to join our small team working 39 weeks a year, 24 hours a week, who will:
• Support the transition of children from home-based education to nursery or school provision.
• Support the inclusion of young children with additional needs and their families in the community in their own right.
• Prepare Educational Health Care Plan reports.
• Facilitate the involvement of parents/carers in the service.
• Work closely with other agencies through liaison and joint visits.
• Participate in regular supervision and team meetings.
• Support the delivery of training for parents and other early years practitioners as required
To be successful in this role, it is essential you have:
• Minimum NVQ Level 3+/professional child centred qualification
• GCSE English and Maths grade 5 or above or equivalent
• Previous experience of working with preschool children foundation stage/key stage 1
• Three years’ experience of working with children with SEN/disabilities and their families
• Experience of working with children and families who need support in being school ready
• Ability to develop good working relationships with parent and children and with other professionals
• Good communication skills across a wide spectrum of parents, children and professionals; knowledge of Makaton/Sign Along/BSL and/or PECS
• Ability to keep accurate reports and records.
• Excellent MS Office skills, including Outlook, Word, PowerPoint and Excel

Starting Salary £13.91 per hour (During probationary period) rising to £14.60 (post probation)
During Probation Period - NJC SCP 10 pro-rata
After successful completion of Probation Period – NJC 13 pro-rata.
