Do you want to know more about Medication for Opioid Use Disorder? This video explains the three primary medications very clearly, it also addresses the myth of "using one drug to replace another" and it helps lower the misperception barrier to getting help. Medication may not be for everyone, but we hope you'll take a moment to learn, share and perhaps open someone's eyes to medication for their opioid use disorder.

There are three approved medications for opioid use disorder (MOUD)...

Congratulations to Gary Pratt, South County Recovery Center on being recognized for his leadership and work in our community!

Good morning! This morning, we spotlight Berkshire 25 honoree Gary Pratt, who has been living in recovery since 2008, which informs his work every day as Executive Director of Rural Recovery Resources. He successfully spearheaded the effort to open the South County Recovery Center in Great Barrington in 2024, coordinating a partnership with Great Barrington Police to fund recovery coaches who assist residents navigating active addiction. Gary launched the grassroots initiative Smash the Stigma 413, focused on dismantling discrimination against people with substance use disorders. He recently launched a new recovery-supportive transportation program to address rural isolation, envisioning on site clinical services and low-threshold, substance use supportive housing in South Berkshire. Congratulations, Gary!!

They say a picture is worth a thousand words, and this weekend there will be approximately 47 million pictures taken of the foliage, every one of them captioned with some version of “peak color” and sent to a relative who lives somewhere with palm trees.

But in EMS, sometimes we have to do the opposite. We have to take what we are seeing, hearing, smelling, measuring and figuring out in the back of an ambulance, then turn it into enough words that the hospital can clearly picture exactly what is coming through their doors. That is an art form.

So today, as the Berkshires show off every shade of red, orange, yellow and gold, we give a shoutout to one of Southern Berkshire Ambulance’s own verbal Picassos. The man himself may be seriously lacking in pigmentation, somewhere between “winter white” and “printer paper,” but when it comes to painting a picture with words, apparently he has the entire 64-count Crayola box at his disposal.

His reports do not simply say, “Patient doesn’t look good.” By the time he is finished, the hospital knows what the patient looks like, what happened, what we found, what we did, what changed, what did not change, and probably what the patient had for breakfast three Tuesdays ago.

So while everyone else is out admiring the fall colors this weekend, remember that some of our finest artwork is being created under fluorescent lights, at 60 miles an hour, with a laptop balanced on somebody’s knee. No easel, no paintbrush, just a radio, a report, and enough descriptive language to make Bob Ross say, “Maybe dial it back a little.”

Ready to Serve. Proud to Care.

📱 THE NEXT SPECIALIST MAY BE CLOSER THAN YOU THINK

When most people picture ambulance care, they think of emergency medications, cardiac monitors and transportation to a hospital. Increasingly, EMS systems are exploring another tool: secure telemedicine.

Ambulance-based telemedicine can allow an EMT or paramedic to communicate with a physician, specialist or other healthcare professional while assessing a patient at home, at an emergency scene or during transport.

Depending on the program and the patient’s needs, that connection may help clinicians:
🧠 Consult specialists about stroke or other time-sensitive symptoms
❤️ Share clinical information with a receiving hospital
🏥 Identify the most appropriate destination or care pathway
🩺 Obtain medical guidance in remote locations
📋 Access relevant health information more quickly
🤝 Coordinate follow-up and community-based services

County Ambulance Service is currently applying for approval to operate a Mobile Integrated Health Care program, commonly called MIH.

MIH represents an important expansion of the traditional ambulance-service model. Instead of waiting until a health problem becomes serious enough to require a 911 response, specially prepared EMS clinicians can work with physicians, hospitals and other healthcare partners to provide coordinated care in the community.

Potential MIH services may include scheduled home visits, post-discharge follow-up, chronic-disease support, medication review, health education and connecting patients with appropriate medical or social resources.

Telemedicine could play a significant role in that expansion. A County clinician visiting a patient at home could potentially connect with a physician or another member of the healthcare team without requiring the patient to travel—bringing additional expertise directly into the home.

Although MIH and community paramedicine are growing, they remain uncommon compared with the traditional emergency-response model. Pursuing this program places County
Ambulance among a relatively small group of ambulance services exploring how EMS can provide not only reactionary care after an emergency occurs, but preventive care that may help keep an emergency from occurring.

Our application remains subject to state approval, and the specific services will depend on clinical partnerships, identified community needs and applicable regulations. The goal, however, is clear: use the people, technology and community relationships already present in EMS to help patients receive the right care earlier.

Technology will never replace personal attention, hands-on assessment or clinical judgment. Used responsibly, it can strengthen those things and help close the distance between patients and the care they need.
