DayBreak exists to serve families tasked with continual care of a loved one diagnosed with Alzheimer’s or other forms of dementia.

A safe place to provide social time for participants while providing respite for caregivers and those who work.

A special thanks to Smoky Mountain Home Health & Hospice, Inc. and Lorri Ryan for helping us collect the information we need to help our families. Next up December 8th Fall Prevention & Home Safety. Join us tomorrow for our next Aging Well & Wise community education session at DayBreak on the Sycamore Tree campus- 1830 Clydesdale Street! It's free, and you can come even if you didn't RSVP. We will serve a light lunch around 12:40-12:45, and our "Getting Your House in Order" session will begin at 1:00pm. Lorri Ryan will walk us through what information we need to organize now to help our families if we are not able to share it. We will also have a funeral planning tool and TN CARETag kits available. Bring a note pad, and we will do the rest. Our Memory Cafe will follow at 2:00 pm.

The TN Department of Health Office on Healthy Aging has a new program called CARETag. The goals are to create a universal identification system, assist first responders, improve emergency planning, and ensure proper documentation and accessibility. https://www.tn.gov/health/healthyaging
