Every person we serve through the CES Waiver Program receives a monthly phone check-in and an in-home visit each quarter (January, April, July, and October). These visits are one component of the program and play an important role in ensuring each person receives safe, effective, and individualized support.
During quarterly visits, our Care Coordination Specialists (CCSs) check safety and medical equipment, update the person’s Face Sheet and Plan of Care, review progress toward goals, and make any needed updates to behavior plans.
But these visits are about more than meeting program requirements. They’re an opportunity to build meaningful relationships with the individuals and families we serve. At The Kistler Center, we want every person and family to know they have a team in their corner—providing support, advocating for their needs, and walking alongside them every step of the way.
