Help People Make a Safe, Confident Transition from Hospital to Home
Are you caring, organised and passionate about supporting people in your community?
We’re launching an exciting new pilot project across Caerphilly County Borough — and we’d love you to be part of it.

💛 About the Role
As a Hospital‑to‑Home Community Connector, you’ll provide short-term practical and emotional support to disabled adults and older people returning home from hospital. Your help will make the difference between someone struggling alone and someone regaining confidence, independence and stability.

👣 What You’ll Be Doing

Visiting people recently discharged from hospital
Identifying immediate practical needs
Supporting with shopping, prescriptions and everyday tasks
Offering reassurance during recovery
Connecting people with ongoing services and community support
Working closely with health, social care and voluntary partners
Helping us demonstrate the impact of this new pilot

🌼 Who We’re Looking For Someone who is:

Experienced in supporting older, disabled or vulnerable adults
Compassionate, reliable and able to work independently
A great communicator who builds trust easily
Committed to promoting independence and wellbeing
