Canadian Frailty Network CFN is a national network dedicated to improving the care of older Canadians living with frailty, and to supporting their families and caregivers.

Canadian Frailty Network (CFN) is dedicated to improving care of frail older Canadians by increasing frailty recognition and assessment, increasing evidence for decision making, and advocating for change in the health and social care systems to ensure that the needs of this vulnerable population are met. We are a not-for-profit corporation funded by the Government of Canada's Networks of Centres o

f Excellence (NCE) program. Our network is built upon the premise that the caring for frail older adults is a complex, Canada-wide issue that requires multi-faceted, national strategies and solutions. To achieve our goals, we partner with universities, hospitals, provincial and regional health authorities, other research networks and organizations, care providers, industry and advocacy organizations to achieve our multidisciplinary approach. We believe in patient-centred care, and older Canadians or their family/friend caregivers are involved in all of our activities.
