Job Description: Community Outreach Support Worker
Start Date: As soon as possible
Hours: 35 hours per week
Wage: $50,960.00 annual salary- 2 weeks of paid vacation
Location: Based out of the Bowmanville Outreach Office (travel required) Reports to: Outreach Supervisor

The Community Outreach Support Worker is responsible for providing trauma-informed, client-centered support to participants in our Outreach program. This role ensures a safe, respectful, and empowering environment for participants as they work toward healing, independence, and self-sufficiency. The worker will support participants in developing life skills, implementing coping strategies learned in therapy, and achieving personal goals. Additionally, this position maintains thorough documentation and collaborates with internal and external supports to ensure continuity of care.

Key Responsibilities Client Support & Case Management
- Conduct monthly 1:1 case management meetings with assigned Outreach clients
- Develop and review individualized case plans and goals
- Conduct weekly wellness checks and provide referrals and support as needed - Assist clients with appointments, errands, and community-based tasks
- Provide safety planning and lifestyle awareness support
- Offer in-person support for Follow Care clients as needed to ensure a supported transition to independence

Program Development & Facilitation
- Contribute to the development of Outreach programming and curriculum
- Facilitate monthly Outreach workshops and activities (therapeutic and recreational)
- Support creation of program tools such as safety kits and best practice guidelines
- Coordinate recognition of client milestones (e.g., birthdays, sobriety anniversaries) through gifts and personal check-ins
- Collect and track data to evaluate program effectiveness and identify emerging needs
- Maintain accurate and timely client records; input notes within 24 hours of each shift
- Uphold privacy, confidentiality, and ethical standards in all documentation and client interactions
- Support collection and distribution of donations and care packages Other Responsibilities
- Participate in team-based approaches to problem-solving and service planning
- Travel within the region to meet client and program needs

Must have a valid G-class driver’s license and access to a reliable vehicle -

Recommended certifications: CPR/First Aid, Non-Violent Crisis Intervention (NVCI), trauma training Working Conditions - Based in the Bowmanville Outreach Office with frequent travel - Occasional evening or weekend hours may be required - Work is performed in various settings, including homes, community spaces, and the office - Employee care benefits available - Medical and Dental benefits

I am learning to embrace a new life, free from substance use and the wounds left by s*x trafficking. I am blessed with the power to choose healing from the degrading and isolating pain of exploitation. For a long time, substance use helped me cope—but eventually, it started taking a toll on me and stopped working.

Now, I am learning to give myself grace as I uncover the calluses that have kept my horrific experiences hidden. As I gently scratch away at the surface, I feel a pain unlike any broken bone or cut. It’s a raw, deep ache—a howl from within that I have denied myself to experience forever. This cry, desperate and primal, sounds like a wounded animal screaming for its life. The wound, once festering and toxic, is now open. I have chosen to give myself the grace of healing.

I am learning to show myself the same kindness and understanding I would offer a dear friend. I draw strength from the courageous survivors who came before me, letting their journeys guide me through this treacherous path to healing. I am giving myself grace by allowing help—help I was once too scared to accept—to coach me and support me at every step.

I am learning to receive the love and kindness I deserve. I give myself grace by embracing my mistakes and setbacks, knowing they are often part of the healing process. And I promise to try again, again and again.

I will recover. I will honor myself and my experiences. I will welcome this new journey of recovery with a willingness of an open heart and gratitude because this choice—to heal—has been denied to so many others who never had the chance to make it.

I will do it for them.

Position Summary
The Team Lead supports the daily operations of the Live-In Program while working directly alongside participants and care staff to provide trauma-informed, culturally safe, participant-centered care within a structured residential setting for individuals impacted by human trafficking and s*xual exploitation.

This is a working leadership role that combines direct participant care responsibilities with day-to-day team support and guidance. The Team Lead provides mentorship and assistance to Care Staff, promotes consistency in service delivery, and serves as a resource during shifts while also independently providing care and support within the home. This position does not include formal management or supervisory responsibilities.

The Team Lead works collaboratively with management, clinicians, program staff, and care staff to support continuity of care and maintain a safe, supportive, and welcoming environment for participants.

Possible warning signs:
🔺 A person not controlling their own identification or money
🔺 Signs of fear, exhaustion, untreated injuries, or malnourishment
🔺 Someone being watched closely or prevented from speaking freely
🔺 Multiple young people being escorted by one controlling individual
🔺 Ads online promoting “quick cash,” private parties, or suspicious recruitment

How to respond safely:
✔ Do not confront suspected traffickers directly
✔ If someone is in immediate danger, call 911
✔ Document details safely if appropriate
✔ Contact local trafficking hotlines or victim services
✔ Focus on survivor safety, not public speculation

Ending trafficking requires prevention, education, housing supports, mental health care, and long-term survivor resources — not just awareness campaigns during major events.
