Grafton, say hello to Amanda 👋 Amanda is one of Harbour At Home’s new Support at Home Care Partners, bringing more than 10 years of experience supporting older people to remain safe, confident and independent in their own homes.

Throughout her career as an Aged Care Case Manager, Amanda has helped clients and families with assessments, personalised care planning, service coordination and navigating the aged care system.

She also works closely with health professionals and advocates for clients to ensure their needs, preferences and goals remain at the centre of their care.

For Amanda, the most rewarding part of her role is getting to know the people she supports — listening to their stories, building genuine relationships and helping them continue living life on their terms.

We’re delighted to have Amanda supporting older people and families throughout the Grafton community.

Port Macquarie - meet Michael and Hannah - Harbour at Home's new local care partners!

Together, they bring more than 30 years of experience across nursing, aged care, community services and home care.

Michael is a Registered Nurse with more than 20 years of experience, including a decade supporting people in their own homes. He is passionate about listening, building trusted relationships and helping people maintain their independence, dignity and choice.

Hannah brings more than 10 years of hands-on aged care experience. She understands that navigating aged care can feel overwhelming and works closely with clients and families to make every step clearer, more personal and less stressful.

Together, Michael and Hannah combine extensive experience with a shared commitment to providing compassionate, personalised care that puts people first.

Community Grants Open!! Following the announcment of Woolgoolga Seniors Centre as the first recipient of our Community Grants Program, we're inviting even more seniors groups to apply!

Our Community Grants Program provides grants of $500 to $5,000 for local organisations across the Coffs Coast, Grafton and Port Macquarie areas that support seniors and strengthen community connection.

NEW GUIDE: How to Access Home Care Funding

Support at Home is now the main government-funded program for older Australians who need help at home - but depending on your situation, it may not be the only funding pathway available.

Now, there's extra funding available for things like:

• Assistive technology, such as mobility aids, shower chairs or other equipment
• Home modifications, such as grab rails, ramps or safer bathroom access
• Short-term restorative care after a fall, illness or hospital stay
• End of life support at home

We’ve created a new guide to help explain what these funding pathways are, who they’re for, and how to access them.

For many people and families, conversations about palliative and end-of-life care can feel difficult to start.

But talking earlier can help make sure a person’s wishes are understood before decisions become urgent.

This week is National Palliative Care Week, with the focus on “Getting to the heart of it: Big Questions. Real Answers.”

For older Australians receiving care at home, this can include questions like:
❓What matters most to me as my needs change?
❓Where would I feel most comfortable being cared for?
❓Who do I want involved in decisions about my care?
❓What support would help my family or carers feel less alone?

This week is a reminder that these conversations do not need to happen all at once. But starting early can make a meaningful difference.
