🤍 MADDY’S MIRACLE GRANT KANSAS
APPLICATION IS NOW OPEN 🤍

🔸WHAT IS IT?

An IVF grant provided by the Hadwin Family Foundation. It is available to residents of Kansas who have financial need and who are patients at the Center for Reproductive Medicine (CFRM)

🔸HOW TO APPLY - STEP 1

Confirm with your physician at CFRM that you meet specific medical criteria prior to applying. The Hadwin Family Foundation will verify with CFRM that you meet these criteria and are eligible to apply.

🔸HOW TO APPLY - STEP 2

Visit our website at https://www.thehadwinfamilyfoundation.org/mmg-application-kansas to apply. Applications must be complete and submitted via the website to be considered.

🔸HOW TO APPLY - STEP 3

Have financial documents available to submit, if requested, to provide proof of financial need. For questions relating to the application, email [email protected]. For medical questions, contact your physician at CFRM.

🔸WHEN WILL A RECIPIENT BE SELECTED?

All applicants will be notified via email, within 7 days after the application cycle closes, whether they were selected or not.
