Unity Care Unity Care ends and prevents homelessness and poverty for youth aging out of foster care.

Unity Care offers a wide range of services benefiting at-risk and foster youth and their families throughout Northern California. Its signature program, established in 1993, focuses on serving the housing needs of youth in foster care and beyond. The organization’s mission has grown and expanded over the past two decades and, today, includes more than two dozen community-based, education and behavioral health programs, aiding approximately 7,620 underserved youth and families annually.

Student Success Spotlight

One of our participants entered the program with a clear goal: earn her college degree and create a stable, successful future for herself.

While balancing academics, employment, and financial responsibilities, she demonstrated remarkable commitment to her education. Through persistence, accountability, and a strong work ethic, she continued making progress toward her goals despite the challenges that often accompany independent living and higher education.

Today, she has successfully passed all of her classes and is only two semesters away from earning her bachelor's degree. This milestone represents years of dedication, resilience, and determination to build a brighter future. 🎓

Beyond her academic achievements, she has strengthened her financial literacy, maintained housing stability, secured employment, and increased her savings as she prepares for greater independence.

You asked. We answered.

As part of Unity Care's opioid awareness and prevention series, we've been listening to the questions youth and young adults are asking about fentanyl, addiction, and overdose prevention.

One question we received was:

Who invented fentanyl and what was it originally used for?

Fentanyl was first synthesized in 1959 by Dr. Paul Janssen and developed as a prescription medication for severe pain management. Today, it continues to be used in hospitals and medical settings under careful supervision.

However, illicit fentanyl has become a significant public health concern. Because it is extremely potent and is often mixed into other substances without a person's knowledge, it has contributed to a rise in overdose deaths across communities nationwide.

Education and awareness remain critical tools in prevention. By answering real questions from youth and young adults, we hope to provide accurate information, encourage informed decision-making, and help create safer communities.

At Unity Care Group, we believe that providing young adults with stability, guidance, and opportunity can create lasting change.

Eric entered our THP+ program seeking support with employment, structure, and long-term career development. While navigating the transition into adulthood, he remained open to learning, growth, and building a stronger future for himself.

With consistent case management support, vocational training referrals, and career readiness resources, Eric stayed committed to his goals. Through dedication and accountability, he successfully completed a vocational training program and secured full-time employment in his field.

This achievement has positively impacted many areas of his life, including increased confidence, financial stability, improved independent living skills, and stronger time management and communication abilities. His progress reflects not only professional success, but meaningful personal growth and resilience.

Stories like Eric’s highlight the importance of combining housing stability with individualized support and workforce development opportunities for transition-age youth.

May is Foster Youth Awareness Month — a reminder to uplift and support young adults as they build brighter futures for themselves. 🏡✨

When one young adult joined our Transitional Housing Program in San Jose, she was unsure about her future career path and lacked confidence in her abilities. With ongoing encouragement and support from her case manager, she began exploring careers in healthcare and discovered a passion for the medical field.

Today, she is enrolled in the Medical Assistant program at IBT Institute College and is working toward her dream of becoming an ultrasound technician. She also took initiative in securing FAFSA assistance and scholarships to help support her educational journey.

Her growth, confidence, and dedication have been inspiring to witness. Stories like hers are reminders of the impact supportive housing, mentorship, and positive guidance can have on young adults transitioning into independence.

We are proud of her progress and excited to see all she accomplishes in the future. ✨

Two Journeys. Two New Beginnings.

At Unity Care Group, we continue to witness the life-changing impact stable housing and supportive services can have on young adults working toward independence.

Two participants in our Scattered Sites Program came to us facing homelessness, financial instability, uncertainty, and a lack of support. Despite these challenges, both individuals remained determined to create a better future for themselves.

Through perseverance, consistent communication with staff, housing searches, property tours, and participation in CPR training hosted by Unity Care Group. Both clients achieved stable housing and continued progressing toward employment and career goals.

Today, they each have a safe place to call home — a space where they can focus on growth, healing, and building the future they envisioned for themselves.

One client proudly shared:
“I’m so happy, I’m going to bring my mom to show her around my place.”

Another reflected:
“Nothing is unachievable if I focus and try my best.”

These stories are reminders that stability creates opportunity, confidence, and hope. We are proud of the resilience, dedication, and progress these young adults have shown throughout their journeys.

At Unity Care, we are committed to helping youth and young adults better understand the risks associated with opioid misuse and addiction while promoting healthy coping skills, prevention resources, and supportive services.

By reducing stigma and increasing awareness, we can encourage individuals and families to seek help earlier and connect with critical community resources.
