In the past week, I've spent over 35 hours fighting insurance denials that never should have happened — so that kids with autism here in Kentucky don't lose their therapy.

Appeals that sat unprocessed. Complaints that went unanswered. Families told to start over — again. I know how to fight this because it's what I do. But most clinics don't have anyone who can, and when no one fights back, children's services get cut off. Not because they didn't need the care. Because of paperwork.

Every week of delay is a week a child can slide backward. That's not an inconvenience. That's harm.

And here's the part most people don't know: these insurance companies get paid for every child on their Medicaid plan — whether they provide services or not. When they delay or deny care, they keep the money.

That's why we started Bluegrass Alliance for Behavior Health — to stand with Kentucky families and fight these delays and denials. And we need your help to keep going.

An update from a very busy week.

First, the why: Kentucky Medicaid isn't making sure kids with disabilities get the services they need.

Since last Monday, we've filed over 400 pages of appeals and grievances for Kentucky families fighting to keep their kids' therapy. And this week we took these issues straight to the people who can fix them:

• We filed grievances with the leadership of the Kentucky Department for Medicaid Services against the Medicaid MCOs — and they've committed to respond by the end of the week. • We asked the offices of Senator Rand Paul and Representative James Comer to help escalate these issues to federal leadership at CMS.

Here's the thing: state regulators say they've had no complaints, so they think nothing is broken. Everyone who works in this field knows better — and the squeaky wheel gets the grease.

Kentucky ABA providers: how many hours did your team spend last month chasing appeals, resubmitting grievances, and re-documenting medical necessity for care a child clearly needs?

How many families have been told their child needs another comprehensive evaluation to prove they still have autism — as if autism goes away?

Clinicians don't belong on the phone. Clinicians belong on the floor, taking care of their clients. Every hour spent fighting a payer is an hour taken from kids — and every delay puts a child at risk of losing services. You shouldn't have to fight this alone, case by case, forever.

We’ve officially launched the Bluegrass Alliance for Behavior Health—and we’re asking for your support.

Across the country, families and providers are navigating a behavioral health system that too often delays, denies, or obstructs care for children with developmental disabilities. Families are frequently forced to fight for diagnoses, treatment authorizations, and medically necessary services that are already required under state and federal law—but inconsistently enforced in practice.

Bluegrass Alliance for Behavior Health was created to help change that.

Our mission is to provide families and providers with direct support to help locate and access care, free education, practical advocacy tools, and system-level accountability—so children can receive behavioral health services without unnecessary barriers, regardless of where they live.

We have now launched our GoFundMe and are officially able to accept donations. Every contribution directly supports:
• Direct support to families working to find and access care
• Free advocacy resources for families facing denials or delays
• Education and training for providers navigating appeals and grievances
• Policy and systems-level work to improve access to care

This morning, I’m heading to Frankfort on ye old West Kentucky Parkway to meet with colleagues and partners to discuss access to behavioral health services for children across the Commonwealth.

We have several meetings scheduled focused on critical issues impacting families and providers, including access to diagnosis and treatment, Medicaid policy, barriers created by commercial insurance practices, and the systemic challenges that delay or prevent care. These conversations will center on practical solutions and how providers, parents, and legislators can work together to improve access to timely, appropriate services for Kentucky’s children.
