What You’ll Do
• Provide nursing assessment, education, and support to Health Home members.
• Help individuals manage chronic health conditions and improve their overall wellness.
• Coordinate care between primary care, behavioral health, specialty providers, and community services.
• Advocate for clients and help remove barriers to accessing healthcare and social services.
• Develop trusting relationships with individuals who may face significant mental health, medical, and social challenges.
• Monitor health needs and help clients follow through with recommended care.
• Collaborate with an interdisciplinary team to develop and achieve individualized goals.
• Document services and maintain accurate clinical records.
What We’re Looking For
• Current Registered Nurse (RN) license in good standing.
• Strong clinical assessment, documentation, and communication skills.
• Compassion, patience, and a genuine desire to help people facing mental health and other life challenges.
• Ability to work independently while collaborating effectively with an interdisciplinary team.
• Strong organizational and care-coordination skills.
• Experience in behavioral health, community health, care management, or working with individuals with complex needs is a plus.

Wage: $28.50 to $35.52 with education and license incentives.
