A group of Service Coordinators shared how they use the Integrated Supports Star in a variety of situations. Primarily, they shared that they use the concept as a conversation starter. Individually, they have used the Star to help a team address sensory needs in a classroom, to develop a 504 plan, identify supports related to a behavior concern, and identify supports to increase physical movement. We’re here at Synergy, and there’s SO much happening! 🎉

On Wednesday, we brought a little fun to the tables with the Charting the LifeCourse (CtLC) Domains Dice Cube! 🎲 at Synergy Exchange

We rolled the dice, saw which Domain came up, and then used it to spark questions and conversations around the table. 💬🗣️ It was such a fun and engaging way to get everyone talking, sharing ideas, and connecting with one another!

Sometimes all it takes is a little roll of the dice to get the conversation going! 🎲😉

An Outreach and Education Coordinator with lived experience uses the One Page Description as an outline for notes when he speaks with people with lived experience. After his 30 minute or so phone call, he says, “this is what I heard you say/what I learned”. He then tells the person that he will give/send it to them and to take time, share with others who help them, add to it and explains how they can use it. PLEASE SHARE SO OTHERS CAN LEARN WHAT IS HAPPENING TO SUPPORT AND EMPOWER OTHERS!

A Service Coordinator shared that a couple of people she works with call her about every concern in their life: physical health, wheelchair breakdown, apartment. She spends time helping them identify WHO to call. She is making an Integrated Supports Star and listing specific supports for the person so they have the info and she can use the Star to support and empower the person to problem solve. She also is going to make a Star with common resources in her area that can help all people. PLEASE SHARE SO OTHERS CAN LEARN WHAT IS HAPPENING TO SUPPORT AND EMPOWER OTHERS!

A Parent Information Specialist shared how she supported a grandmother/caregiver to develop a One Page for her grandson. He has significant medical issues and was changing schools/teachers. They developed a One Page specifically related to medical concerns, laminated it and put it in his backpack. They also shared a copy with his teachers. The Grandmother felt more assured that everyone had or could easily access the info, if needed, and know how to support her grandson. It is empowering to know you have done all you can! Several Parent-Youth Ambassadors (PYAs) shared the impact of using the Planning Trajectory with different people and families. One PYA used the Planning Trajectory to support a woman who was the victim of human trafficking. The framework and tool helped the woman see how much she has done and what she can do now-enhancing her self-esteem and self determination! Another PYA shared that the Planning Trajectory helped a family see all that they had accomplished when they felt stuck and unsuccessful!
