A Grandmother/Caregiver shared how powerful using the Planning Trajectory with her teenage grandson was to support him to think about life after High School. He came to live with her over a year ago and his “vision” was to quit school. He had experienced a lot of trauma. After a year of supports to address trauma as well as a supportive loving home, she again had a conversation with him about his vision of life. They reflected on his past and how he has moved forward. He is currently looking at colleges and is positive about his life and future! PLEASE SHARE SO OTHERS CAN LEARN WHAT IS HAPPENING!

A Parent-Youth Ambassador (PYA) was contacted by a mother who was overwhelmed because her husband left her and their children. The PYA used several tools, including the Vision Tool, Trajectory and Integrated Supports Star to frame her conversations and record mom’s thoughts. The mom’s initial thought was, “I don’t know what I will do!” After she saw her thoughts written down, she saw what she wants/her vision and the supports she has as well as what she has to do next, including get legal counsel, she felt hope, organized and supported.

A Service Coordinator (SC) is working with a student who will graduate this year. He does not want to leave school and is afraid to go somewhere new. He is currently exploring Day Programs and is anxious to leave the comfort of school. She worked with him and his school staff to develop a One Page Description that he can share when he goes to a Day Program. She emphasized that this is a way that he can share what a great guy he is and how he wants them to help him. He enjoyed developing it and hearing what his school staff said about him! He shared that his feels better knowing that new staff will know about who he is! The POWER of the ONE PAGE for his self-esteem and comfort in a new environment with new people!

A Parent Mentor and parent shared how she shared the Caregiver Well-Being Portfolio with parents supporting them to think about taking care of themselves. The session was very emotional and families shared how they DO NOT think of themselves. This made her aware of how she does not think of herself and forced her to go home and have a conversation with her family about what they can do to take care of themselves. She shared that she had to ‘let go’ and ‘trust’ others. She is excited about how this impacted her personally and helped her identify HOW to take care of herself. She is also excited to meet with the parents again and see how they have used the info and the impact on their lives!

A Service Coordinator (SC) shared that she was working with a young man in his late 20s. She used the Planning Trajectory to facilitate their conversation. He shared so many goals that the SC decided to record them on big paper that she put on the wall. They then looked at how to prioritize them so they could move forward. The young man asked for the paper-he wanted to put it up on the wall in his bedroom so he could look at it, continue thinking about his life and stay focused. The paper had meaning to him-his thoughts and vision. The POWER OF THE PAPER!

A Service Coordinator (SC) is working with a 7 year old who has a medical condition that causes him to experience pain if he is moved or positioned incorrectly. He is very apprehensive about being around new people and places. She worked with him and his family to develop a One Page Description for medical appointments focusing on how professionals can move his arms and body during testing or exams and NOT CAUSE HIM PAIN. He shared that he feels better that the info is written down and they can give the medical professionals his One Page when he goes to appointments. Remember that the ONE PAGE is written for a purpose and for the person reading it!
