Expertise: Bringing together world leaders in the care of critically ill patients for collaboration to advance quality of care through education and publication. Clinical support: Maintaining a comprehensive registry of data to assist in reducing morbidity and improving survival of patients requiring extracorporeal therapies. Community: Fostering communication and collaboration among professionals who apply advanced technologies in the treatment of refractory organ failure.

Long-term mortality & functional outcomes among adults treated with
๐Ÿ” systematic review/meta-analysis including 163 studies (RCTs + observational studies) and >78K patients
๐Ÿชฆ long-term mortality post remains high and continues to accrue over the long-term, with substantial variation by modality/clinical indication, ranging from 37.2% for VV to 74.4% for at 1 year, but heterogeneity between studies remains substantial
๐Ÿ“š functional outcomes (inconsistently) reported only in 23.3% of studies, including >7.8K survivors using CPC, WHODAS 2.0 or mRS, limiting the understanding of functional recovery among survivors
Standardized outcome reporting, and greater focus on survivorship and patient-centered outcomes in future research pivotal to better inform long-term prognosis/follow-up, patient selection, and factors that may improve functional recovery after support.

Left ventricular unloading during VA in patients with refractory cardiogenic shock:
๐Ÿ” target trial emulation analysis including 264 patients on VA for โ‰ฅ48h in 2 high-volume centers, 2019-2023
๐Ÿซ€ 52.2% received VA-ECMO alone, 47.8% ECMO + LV unloading ( or microaxial flow pump within 24 h after ECMO initiation)
โš–๏ธ early addition of LV unloading not associated with survival benefit at 60 day, nor with improved ECMO weaning rates, despite an increased incidence of device-related complications
Findings suggest that a non-selective approach to LV unloading may expose patients to unnecessary risks without improving outcomes; clinically meaningful benefits in carefully selected patient phenotypes cannot be excluded. Results of ongoing RCTs (UNLOAD-ECMO, EULODIA,EASE-ECMO, REMAP-ECMO) eagerly awaited to define optimal unloading strategies: indications, timing, modality.

๐Ÿ“ข The 2025 Annual Report of the ELSO is now available on our website!! The Report reflects the continued growth of the organization, the science and practice of , and the growing bonds among our many chapters, clinicians and partners throughout the world. Unfortunately, it was also the year that we lost our founder, Dr. Robert H. Bartlett on October 20, 2025. This marks a new era, not just for ELSO, but for the entire field of extracorporeal life support. It is hard to overstate Dr. Bartlett's intluence. His impact spanned from scientific discovery to the mentorship of generations of students. Dr. Bartlett taught us all to be curious and humble, to pursue science in the interests of our patients. He was also a friend to so many of us throughout the world. While we celebrate ELSO's achievements of 2025, we look to honor Dr. Bartlett's legacy as we continue our work into the future by challenging ourselves to innovate, to inspire and to serve, as Dr. Bartlett always challenged us to do. Thank you for all you do every day.

โš–๏ธ median BMI 31kg/m2
๐Ÿชฆ in-hospital mortality 62%; each 1 kg/m2 increase in BMI independently associated with 6% higher odds of in-hospital mortality in non-COVID ARDS
โš–๏ธ higher BMI associated with renal complications across ARDS etiologies
โš–๏ธ higher BMI associated with infectious complications in non-COVID ARDS, with graded increases across WHO obesity classes
Findings challenge the obesity survival paradox in respiratory and support modeling BMI as a continuous risk factor for VV ECMO risk stratification, counseling, and complication mitigation. Absence of a futility threshold reinforces that BMI alone should not be considered absolute exclusion criterion for VV ECMO.

The unknown "others" of neonatal respiratory
๐Ÿ” review of the ELSO Registry data on neonatal respiratory support aimed to reclassify "other and PPHN" patients into modern categories to improve understanding of indications & their diagnosis-specific outcomes
๐Ÿซ >3.2 K neonates included, Jan 2017 - Jul 2021, 41% = "other", 8% persistent pulmonary hypertension
๐Ÿ”Ž survival was 74% for "other" group
๐Ÿ”Ž following reclassification, survival ranged from 0% for alveolar capillary dysplasia to 92% for aspiration of non-meconium fluid.
As ECMO use continues to provide an important life-saving therapy for critically ill neonates with cardiopulmonary failure, more diagnosis-specific data needed to better understand outcomes. In April 2023, ELSO introduced a new workflow for entering the primary diagnosis for neonatal respiratory , with the goal of improving diagnostic accuracy and precision.

Preoperative stabilization before surgery in patients with cardiogenic shock secondary to valvular dysfunction: moving from traditional VA to LAVA
๐ŸŒ‹ case series including 4 high-risk patients with acute severe mitral regurgitation or obstructive aortic stenosis
๐Ÿ” all patients survived to hospital discharge, despite an extreme predicted operative risk according to STS-PROM scores.
In carefully selected patients with in acute valvular failure, LAVA-ECMO may be an effective rescue strategy, combined with a multidisciplinary heart team approach. Principal advantage of left atrial venoarterial ECMO may extend beyond circulatory support to include effective left-sided decompression, particularly when rapid stabilization is required as a bridge to definitive surgical valve replacement.
