Membership Forms Are Now Live
Thank you for your patience while we worked through the details. Our goal was to create a process that is simple for you, while also ensuring we can quickly access the important information provided from our phones —especially in the event of an emergency.
We now have two forms:
* Participant Membership Form – to be completed by the primary caregiver (parent, guardian, or POA) for anyone attending Maryland Cool Kids events or activities https://omella.com/6nxos
* Staff Membership Form – to be completed by all direct support professionals attending events or activities, including family as staff. https://omella.com/86nkf
These forms are only required for individuals who attend in-person events or activities. You do not need to complete a form to remain part of our Facebook group.
We’re calling this a free membership because we thought it would be fun for our participants to have official membership cards—they are the inspiration behind everything we do.
In reality, this membership helps us continue to grow while keeping everyone safe, organized, informed, and in compliance with our insurance.
What the Participant Form Covers:
* Basic & Emergency Information
This ensures we have essential details and contacts in case a staff member experiences an emergency and we need to step in to support a participant.
* Community Support
Our programs are powered by donations and a small but dedicated group of unpaid volunteers. Currently, a handful of people support over 25 hours of programming each month—plus countless behind-the-scenes hours spent on planning, fundraising, bookkeeping, securing locations, and managing communications.
To continue offering free and low-cost activities, we need community support. This section simply outlines ways you can help, based on your availability and interests.
* Rules & Liability Waivers
This ensures everyone understands expectations and helps us maintain a safe, respectful environment. It also keeps us in compliance with our liability insurance.
Staff Form:
This is a shorter form that includes emergency contact information, along with rules and liability waivers.

Important Dates:
* We will begin checking for completed forms on May 11
* Forms will be required for all regular attendees by May 25
* New participants will be given the same grace period
The form takes about 5–6 minutes to complete and can be done easily from your phone.

Please Note:
There will be no exceptions. Anyone who wishes to continue attending our free or low-cost events must complete this one-time form.
Outside of this requirement, nothing else is changing. Participants who have completed the form, and have one-to-one staff, can continue to attend events as usual and will always be welcome in the Cool Kid Community.
