Phil (not his real name) was known to HAB following a period of rough sleeping and unstable accommodation placements.
Prior to coming to HAB, he was experiencing significant substance misuse issues, alongside long-term housing instability.
His situation was further complicated by poor mental health and limited engagement with services, making it difficult for him to sustain accommodation or maintain consistent support.
Upon engagement with HAB, Phil was supported through intensive keywork focused on stabilisation, engagement, and access to appropriate services. HAB successfully secured accommodation for him through the Single Homeless Project, providing a structured and supported housing environment.
Alongside securing accommodation, HAB staff worked closely with Phil to address his substance misuse needs.
Through consistent engagement, encouragement, and motivational support, he was successfully linked in with Change Grow Live (CGL), enabling access to drug and alcohol services, harm reduction support, and ongoing treatment pathways.
This multi-agency approach was key in helping Phil begin to stabilise both his housing and health circumstances.
Since moving into supported accommodation with SHP, Phil has made positive progress in engaging with his recovery journey.
He is maintaining his accommodation, attending key appointments, and continuing to work with support services.
While recovery remains ongoing, there has been a clear improvement in stability, engagement, and overall wellbeing.
