Completing a developmental questionnaire helps answer questions about how your child is learning and growing. Families with children between the ages of 2 months and 5 years old living in Macomb, Oakland and Wayne Counties can complete a FREE Ages & Stages (ASQ) developmental questionnaire. It only takes 10 minutes to complete, and an ASQ Developmental Screening Specialist will send you your child’s individualized results, learning activities and resources.
