Answer B: Discuss goals of care with the patient and family, initiate a palliative approach focused on symptom control, and document preferences in the chart

In the PALTC setting, comprehensive care emphasizes quality of life, respect for patient preferences, and avoidance of unnecessary hospitalization. Given the patient’s expressed wishes and progressive condition, initiating a goals-of-care conversation and transitioning to a palliative approach is most appropriate.
