Navigating behavioral health services can be complicated and feel overwhelming. With the support of a Blended Case Manager, it doesn't have to be. Interact’s Blended Case Management (BCM) program helps Philadelphia residents with behavioral health needs to connect with the services and resources they need to move toward their goals. Interact's BCM program offers support for adults, children, adolescents, and their families.
BCM is a voluntary, community-based service that provides personalized support wherever it’s needed—at home, at school, in the community, at appointments, and more.
Our case managers can help participants with obtaining:
• Behavioral health and medical services
• Housing, benefits, and community resources
• Crisis intervention and hospital transitions
• Employment, life skills, and wellness
• Educational advocacy and IEP support
• Transportation and legal supports
• Self-advocacy skills
• And more
Each participant works alongside their case manager to create person-centered S.M.A.R.T. goals, while our team collaborates with families, providers, and other supports to help participants stay connected to services and make meaningful progress.
🌟 Adult BCM: Ages 18+
🌟 Child & Adolescent BCM: Up to age 18, or up to 22 for eligible high school students with an IEP
