The Community Care Van will be providing FREE services, including:

• Blood pressure screening, care and education
• Diabetes screening, care, and education
• Colorectal cancer screening and education
• Flu + COVID vaccines

You do not need to be a Mass General Brigham
patient to receive services. We do not ask about
your immigration status.
Do you have questions or concerns about your
blood pressure or diabetes? Come talk to us! We
can help answer your questions.
