This, combined with a decrease in the number of professionals entering case management, presents a challenge for the case management industry. Are you—or your team—using innovative approaches, technology, or evidence-based strategies to improve care for people with insulin-dependent diabetes?

The CMSA Foundation Case Management Practice Improvement – Insulin Dependent Diabetes Management (CMPI-IDDM) Award, sponsored by Insulet, recognizes outstanding case management programs that demonstrate meaningful improvements in care and measurable patient outcomes.

Your project could involve:
🔹 Diabetes technology and data integration
🔹 CGMs, insulin delivery systems or smart pens
🔹 Treatment adherence strategies
🔹 Process improvements that lead to better outcomes
🔹 Other innovative approaches to insulin-dependent diabetes management

Established to recognize chapters that demonstrate exceptional commitment to advancing the Foundation's mission through fundraising, promotion, creativity, and member engagement, this award celebrates the meaningful impact chapters can make within the profession.

The CMSA MidAmerica Chapter exemplified that mission through innovative and sustained support of the Foundation—including a successful Walk-A-Thon, conference fundraising activities, ongoing Foundation awareness efforts, and a unique professional book initiative. Together, these efforts raised more than $6,000 while helping advance education, research, innovation, and engagement across the case management community.
