One of the most overlooked dimensions in modern psychotherapy is the moral dimension.
Character, virtue, conscience, responsibility, integrity. So much of how people carry suffering, guilt, forgiveness, and growth lives here β€” yet few of us were trained for it.
The research: a 2022 systematic review of 57 studies linked moral injury β€” the lasting weight of guilt, shame, and betrayal β€” to depression, PTSD, and suicidality, with clinicians noting it often calls for moral repair, not symptom-focused treatment alone (Hall et al., 2022).

Spiritual β€” "The longing for more"

For many clients, faith isn't a side topic β€” it's a source of strength.
Prayer, meaning, transcendence, grace, vocation. Just as mindfulness moved from contemplative roots into mainstream care, the spiritual dimension can be honored as a genuine clinical resource β€” not a conversion goal.

The research: a 2021 meta-analysis of longitudinal studies found regular religious service attendance was associated with roughly a one-third lower risk of developing depression (Garssen et al., 2021; VanderWeele, Harvard).
