Professional Case Coordination Services continues to grow and provide quality supports to individuals with developmental differences. We hire only well qualified caring and devoted persons dedicated to doing what is necessary for the individuals we serve. As we grow, we will be searching for qualified individuals to join our team. Either looking to change where you work now in the field or currently working in this particular profession. We may be where you want to be. PCCS is the highest compensating agency then any other in the state of New Mexico and where we continue to be one of the best and continue to provide the best. Qualifications are:
Masters or Bachelors degree in a social science.
***Associates Degree accepted under exception process***
Social work degree.
RN with a current license.
6 years case management or Case Coordination experience in related field working with individuals with developmental differences or mental health.
Reliable transportation. Must be willing to travel.
Must be able to pass full background check prior to working.
Must be a self starter and self motivated to complete the tasks timely and adhere to timeframes.
Must be comfortable with working from home with some in office meetings and or virtual meetings and conducting visits monthly with each person on the caseload.
Strict adherence with all DOH regulations/standards, Medicaid and other State entity regulations related to the care of an individual with developmental differences or mental health.
Computer skills a must.

Specifics:
Case Management tools required:
Computer, supplies, insurances, phone, fax, internet, reliable transportation, PPE etc. all will be discussed in interview.

Current services provided by PCCS:
DD Waiver Case Management
Supports Waiver Community Supports Coordination

(Pending in process) Mi Via Waiver, Medically Fragile Waiver and Guardianship.

Specific trainings will be discussed at the interview.

Applicant will be a sub-contractor. Compensation depends on caseload.

PCCS serves three of five large regions of New Mexico.
