Credit where credit is due, well done Sarah and Abbie.
Lovely words from the Care Manager at a home in Halifax where Sarah and Abbie had taken a patient on our Seamless Home from Hospital service.
"I would like to give some well deserved recognition to Sarah and Abbie for the outstanding care and professionalism they demonstrated when bringing MP, a patient living with dementia to our care home. They went above and beyond to make a challenging situation as calm and manageable as possible.
It was MP's first day and she found the transition very difficult and became verbally aggressive and distressed. Throughout the situation both Sarah and Abbie remained incredibly calm, compassionate and professional, taking everything in their stride , providing reassurance and support to both me and the staff helping MP to settle and feel more comfortable.
They liaised with the hospital, kept everyone informed and also supported MP's daughter who was visibly upset by her mum's behaviour.
They demonstrated exceptional practice, patience and compassion, treating MP with dignity. Their professionalism and teamwork was truly impressive.
They are a credit to the service and deserve recognition for the excellent care they provided".
