Community Wellness Coordinator

Wage: $23.00-25.00/hour
Hours: 37.5 per week
Reports To: Executive Director
About Us
The Mission Friendship Centre Society provides culturally appropriate programs and services to support the health and wellbeing of the urban Indigenous community. We are seeking a compassionate and organized Community Wellness Coordinator to deliver health awareness, prevention programs, and mental health/diabetes education.
Position Overview
The Community Wellness Coordinator is responsible for providing health-focused programming with an emphasis on Diabetes and Mental Illness, supporting individuals in the urban Indigenous population, and building connections with community health services.
Key Responsibilities
Health Education & Awareness
Increase public knowledge of Mental Illness and Diabetes through weekly Community Kitchen programming focusing on healthy nutrition.
Deliver monthly workshops on Mental Illness and Diabetes awareness.
Provide one comprehensive diabetes-focused workshop covering prevention, onset, and management.
Mental Health Support
Facilitate weekly psycho-educational group sessions in partnership with Mission Mental Health Unit and/or community partners.
Provide a minimum of 10 hours per week of individual mental health support, assessment, and referrals.
Assist clients in recognizing mental health concerns and accessing appropriate services.
Build and maintain service linkages with Mental Health services throughout the Fraser Valley.
Program Delivery & Cultural Integration
Create and deliver health workshops based on current best practices (e.g., diabetic cooking, chronic disease, active living, mental wellness).
Organize monthly cultural activities and ensure culture is incorporated into all programming.
Coordinate age-appropriate activities for clients attending the Centre.
Promote and participate in cultural events and community workshops.
Administration & Reporting
Maintain accurate client files, records, and progress notes (written and electronic).
Track attendance and evaluate program effectiveness.
Conduct pre- and post-program surveys.
Collect accurate statistics and submit monthly/quarterly reports to the Executive Director.
Assist with Centre events and attend conferences as required.
Skills & Abilities
Strong communication skills; caring and respectful approach.
Ability to multitask, meet deadlines, and work under pressure.
Competent in Microsoft Office (Word, Excel, Outlook).
Ability to work independently and as part of a team.
Reliable, punctual, friendly demeanor.
Knowledge & Experience
Certificate/Diploma in Social Services OR 2–3 years related work experience.
Experience working with individuals from various socio-economic backgrounds and across all ages.
Understanding of the Residential School System and its impacts.
Experience delivering workshops or group programming is an asset.
