Here at Community Health Centers of Southeastern Iowa, our mission is to provide affordable, comprehensive, culturally appropriate, cost effective primary health care to residents of greater Southeastern Iowa region. Especially those individuals/families with limited resources or with other barriers to health care, in order to improve their overall health status. Our vision is to be a primary care

based organization, promoting improvement in the health status of the area residents through emphasis on coordinated care of our patients chronic and acute illness treatment, along with health education and health promotion services.

CHCSEIA is offering an Addiction Recovery Support Group at our Keokuk Clinic. The meetings are hold every Tuesday and Thursday from noon-1pm at the clinic. No registration is required. Everyone is welcome. This is a supportive, confidential, judgment-free environment. You can share your experiences, build coping skills, and find encouragement on your recovery journey.

Due to a data breach affecting 1.4 million beneficiaries, Medicare is issuing new cards with new Medicare numbers to help protect and prevent fraud. This does NOT change your coverage or benefits.
Keep using your current card until June 17th. After June 17th, use your new card and destroy the old one. Let your providers know you have a new number.

If you open a letter with a plastic Medicare card in it, don't "activate it." This is a scam. Medicare is not sending plastic Medicare cards. Report this to SHIIP/SMP at 1-800-351-4664.
