Our team was honored to attend the Albany Guardian Society’s Annual Elder Abuse Resource Fair, where we connected with community members, caregivers, and fellow organizations dedicated to supporting older adults.

Events like this remind us how important it is to come together, share resources, and raise awareness about issues affecting our aging population. We engaged in meaningful conversations, answered questions about our programs and services, and gained valuable insights into the needs of individuals and families in our community.

Thank you to the Albany Guardian Society for bringing organizations together to educate, advocate for, and support older adults. We are grateful for every opportunity to build relationships and help connect people with the resources they need to remain safe, independent, and empowered at home.

Exciting News!

Consumer Directed Choices (CDChoices) is proud to announce a new partnership with Healthy Alliance, the designated Social Care Network (SCN) Lead Entity for the Capital Region, Central New York, and North Country under the New York State (NYS) Medicaid 1115 Waiver Program. This collaboration marks a significant milestone as CDChoices continues to expand access to community-based services that address the social factors affecting health and overall well-being.

Through this partnership, CDChoices will deliver Screening, Navigation, and Enhanced Care Management services to Medicaid-eligible individuals throughout the region. These services are designed to identify and address Health-Related Social Needs (HRSNs), including food insecurity, housing instability, transportation barriers, and other challenges that can directly impact health, stability, and independence.

Working with Healthy Alliance, CDChoices will connect eligible Medicaid members to Enhanced HRSN Services and provide navigation support to help individuals access critical state and local resources. In addition, CDChoices will deliver Enhanced Care Management, offering a coordinated, community-based approach that strengthens care planning, supports self-sufficiency, and improves overall health outcomes.

Together, Healthy Alliance and CDChoices are committed to strengthening the integration of health, behavioral health, and social care services across the region. By leveraging Healthy Alliance’s extensive network of healthcare providers, community-based organizations, and social service partners, this partnership will help ensure Medicaid members can access the supports they need to thrive.

We had a wonderful time celebrating at the 20th Annual Say It Out Loud BIPOC Pride event, hosted by In Our Own Voices!

Today, the Consumer Directed Choices team had the pleasure of connecting with seniors at the Summit at Mill Hill through an educational presentation focused on home care services. We shared information about our Expanded In-Home Services for the Elderly Program (EISEP), Private Pay services, and in-home Respite Care options available to support older adults and caregivers.

At Consumer Directed Choices, we are proud to support Men's Health Awareness Month and the importance of proactive care, early detection, and overall well-being.

Our Team wore blue to show their support and help spark conversations about men's health and wellness, including the importance of staying on top of routine care and mental health check-ins. This month is a reminder for the men in our lives to prioritize their health, both physical and mental, through regular checkups, preventive screenings, and consistent self-care. Taking early action can make a meaningful difference in living a longer and healthier life.
