Consumer Directed Choices (CDChoices) is pleased to announce a new partnership with Public Health Solutions (PHS), one of New York City’s designated Social Care Network lead entities under the New York State (NYS) Medicaid 1115 Waiver program. This partnership focuses specifically on improving access to nutritious food and reducing food insecurity, both of which are critical social determinants that directly impact health and overall well-being.
The NYS Medicaid 1115 Waiver Program supports individuals of all ages experiencing challenges such as housing instability, food insecurity, transportation barriers, or caregiving difficulties. As part of this initiative and working alongside PHS, CDChoices will help expand access to nutrition-related supports for eligible Medicaid members in Kings (Brooklyn), New York (Manhattan), and Queens counties, further advancing New York State’s efforts to address health-related social needs through community-based services.
Nutritional services include clinically and medically tailored home-delivered meals, food prescriptions, and pantry stocking with fresh produce, non-perishable groceries, and essential cooking supplies. These supports are designed to improve access to healthy food, promote better health outcomes, and help individuals maintain greater stability and independence in their daily lives.
PHS, a leading nonprofit organization in New York City, has been at the forefront of addressing social determinants of health through public health programming, benefits enrollment assistance, and community-based service coordination. As a Social Care Network lead entity, PHS brings extensive experience in cross-sector collaboration and care integration, supporting a robust network of providers and community partners working to improve health equity across the city.
