We are delighted to share our findings from the 2025 Grantee Perception Survey, a tool we use to measure our effectiveness as a trust-based funder. Using a third-party evaluator, the most recent report gleaned and interpreted feedback from 25 recipients of FCH funding between 2023 and 2025. Key findings from the report included high levels of trusting, responsive relationships with FCH staff and significant interest in more beyond-the-grant support.

Rural Transformation Grants Awarded in CT and NY
The Rural Health Transformation Program, funded through H.R.1, awarded Connecticut with $145M and New York with $212M to improve health care and access in rural communities. Both states relied on input from stakeholder organizations and community partners to inform their proposal’s initiatives. Core elements of each proposal included building rural healthcare workforce, investing in technology and virtual care, and expanding access to preventative and primary care.

With median sales prices of single family homes well above $700,000 in many Litchfield County towns and hundreds of households on waiting lists for the affordable rental homes that exist, LCCHO addresses the need for affordable owner and renter housing options by providing deep and ongoing support to the local efforts to create them. Beyond $1.1M in grants to 36 partners, we made our second place-based investment—a $1M loan supporting a collective of farms establishing a regional food hub—and provided 156 hours of hands-on support to strengthen the work of our nonprofit partners. The road ahead is uncertain, but we’re committed to using what we have—resources, flexibility, and time—in service of the communities we serve.

Investing in the health, well-being, and equity of 17 towns in eastern New York and northwest Connecticut.
