ClearPath Healthcare

ClearPath Healthcare While our name has changed, our dedicated team, our commitment to 24/7 care, and our deep roots in Central Oregon remain the same.

Offering hospice care, transitions, veterans support, bereavement, and now in-home primary care throughout Central Oregon. Hospice care is designed to focus on quality of life for the terminally ill patient. Our goal is to enable patients to be as comfortable and as free of symptoms and pain as possible, so that they live each day as fully as possible. We believe in treating the patient holistical

ly, honoring their life and their wishes. Our philosophy is to provide support for the patient’s emotional, social, and spiritual needs as well as medical symptoms management as part of treating the whole person. We incorporate a small team approach in providing compassionate personalized care for each patient. Our primary focus is our patients and their families, who we serve with dignity and respect. We believe that our patients have a right to participate in all decisions about their care. We offer suggestions based on sound medical practices but it is our patient’s life and ultimately our patient’s choice as to what will help them on this very personal journey.

If finding a primary care provider has been on your list, we want you to know: we are welcoming new patients!

Our clinician, Taylor Garr-Jones, FNP-C, comes to your home to provide primary care, taking her time with each visit. Taylor manages chronic conditions, reviews medications, orders labs, and coordinates with your specialists. And because we know caregivers often forego their medical needs, Taylor can see them too, blocking two appointments during the same home visit.

There are no membership fees or enrollment costs, and your care is billed directly to Medicare or private insurance. Like a traditional clinic, copays may apply.

If you are 55 or older, managing complex medical needs, or just having trouble getting to the doctor's office, this is for you. Give us a call or sign up on our website today.

For 47 years, we've had the privilege of being welcomed into people's homes across Central Oregon. And that trust has never been lost on us.

During this time, our teams were quietly learning something powerful: care feels different when it happens in someone's own kitchen, bedroom, or favorite chair. The familiar surroundings matter.

Our In-Home Primary Care program is new, but the approach behind it isn't. It's built on decades of experience in this community.

If you or someone you love could benefit from healthcare that comes to you, we'd be grateful to connect with you. We're currently welcoming new patients in Deschutes, Crook, and Jefferson counties.

We see some familiar faces! We love Leadership Redmond, and all of the community connections you can make in this program. ❤️🎉🥳

Congratulations to the Leadership Redmond Class of 2026!

Over the course of the program, this class got an inside look at the people, places, and systems that keep Redmond and Central Oregon moving. From local government and healthcare to the airport, nonprofits, public safety, education, and economic development, Leadership Redmond gives participants a clearer understanding of how decisions are made, how services connect, and how community progress happens.

Programs like this matter because strong communities need people who understand the full picture. These graduates showed up, asked questions, built relationships, and took the time to learn how Redmond works from the inside.

We’re proud to celebrate the Class of 2026 and look forward to seeing the ways they continue to lead, serve, and strengthen our region.

We have a special need...

Every June, campers who are carrying the loss of someone they love spend a few days at Camp Sunrise. We are grateful to all our volunteers who make this camp so special. Right now, we are two men short of having a full camp staff. If you have a heart for kids and enjoy sharing a campfire, kicking a soccer ball, or just listening, we need you.

We provide training; experience working with children is preferred, but not required. What matters most is patience, warmth, and a willingness to show up. A background check is completed for each volunteer.

Camp Sunrise runs June 19 to 21 at Cascade View Retreat in Powell Butte, and our team will support you every step of the way.

If this is something you have been looking for, we would love to talk with you.

We know that traveling to a doctor’s office isn’t always easy. That’s why we began offering in-home primary care throughout Central Oregon. Our goal is to make healthcare simple, accessible, and stress-free for you and your loved ones.

If being seen by a primary care provider has proved challenging, we'd be honored to step in and walk alongside you. Our in-home primary care model is structured so that both our provider, Taylor Garr-Jones FNP, and our Licensed Practical Nurse go to your home together for every visit.

Taylor is a board certified Nurse Practitioner with a clinical focus in geriatric healthcare, hospice, long-term care and in-home visits. She has experience treating patients in geriatric settings and managing chronic conditions. Learn more about our program here: https://f.mtr.cool/gzlvuzhvdq

For 47 years, ClearPath has made one decision over and over again. Patients first.

Our nonprofit status allows us to prioritize community impact over corporate margins, ensuring our budget is always aligned with our mission. That is why ClearPath holds a 5-star Medicare rating, the highest possible. It is why 98% of families we serve say they would definitely recommend us.

It is also one of the reasons ClearPath leadership removed the membership fee for our In-Home Primary Care. We do not want our friends and neighbors to struggle with access.

Patients first. Every time.

Last Friday, we paused to honor those who served.

New names were added to our Veterans Wall of Honor — each one is a life, a story, a family. We are so grateful to hold this space for our community.

If you know someone who should be added to our Veterans Wall of Honor, there is no longer a charge to have a name engraved on our granite panels, thanks to a generous donation. No appointment is necessary to view the Wall of Honor - it is located at ClearPath Healthcare at 732 SW 23rd Street Redmond.

Memorial Day has passed, but for the veterans still with us, being seen and honored does not end with a single day.

The Wall of Honor at ClearPath Healthcare is the only veterans remembrance wall of its kind in Central Oregon. It stands because the men and women who served deserve to be seen, honored, and never forgotten.

Our Serving Those Who Served program offers specialized recognition and support to veterans and their families across Central Oregon.

If you know a veteran who could use companionship, care, or simply someone to listen, we are here.
