Attention Spartan Athletes! Get your physicals and support Sports Med!

Cost: $10 per student (CASH ONLY). We cannot accept credit or debit cards for this event.

Giving Back: 100% of the proceeds will be donated directly to your school’s sports medicine program, provided that 10 or more student athletes participate.

Required Documentation: Please bring all necessary physical forms with the Health History section completed in advance. Please advise parents that we cannot view Dragonfly forms so we need these documents printed.

Eligibility: This event is open to all middle and high school student-athletes.

Parental Consent: Any athlete under the age of 18 must be accompanied by a parent or legal guardian or bring our signed Minor Consent Form.
