Identifying gaps in care (missed screenings, medication issues, follow-up appointments)

The Care Management Team is a group of nurses, social workers, care coordinators, and benefit specialists whose job is to help you and your family navigate complicated or ongoing medical situations. You can chat with them online or by phone and they will get you help for whatever you need.

White said. “Building and modernizing classroom-to-career facilities means good construction jobs for our members right now, and a stronger workforce pipeline for years to come. We know Helena is the person who can actually get this done, and we are proud to stand with her and support her for governor.”

Helena Foulkes outlined a portion of her economic plan which proposes a $100 million bond to fund classroom-to-career facilities.
