While Nigeria has excise taxes on to***co, alcohol and sugar-sweetened beverages (SSBs), current tax levels and structures remain below international benchmarks, while revenues are not systematically linked to health financing. At the same time, the country is facing a growing NCD burden, significant out-of-pocket health spending, and continued pressure to mobilise sustainable domestic resources for health.

The ongoing legislative discussions around SSB taxation, alongside broader fiscal reforms, present an important policy window to ask a bigger question: How can health taxes be designed and implemented to both improve health outcomes and contribute to sustainable health financing?

Today, a consortium of partners comprising the Health Strategy and Delivery Foundation (HSDF), DGI Consult, and the National Health Insurance Authority (NHIA), with support from the Alliance for Health Policy and Systems Research, is convening an inception workshop on โ€œRealizing Health Taxes for Sustainable Health Financing and Curbing NCDs in Nigeria.โ€ As part of the workshop, Dr. Yewande Ogundeji, Managing Principal at HSDF, is presenting on Nigeria's health tax landscape, current challenges and emerging opportunities for reform.

The workshop brings together policymakers, legislators, revenue and budget institutions, development partners, civil society, academia and other stakeholders to identify practical pathways for advancing health tax reform, while launching the Health Tax Task Team to support continued coordination and action.

This is an opportunity to strengthen health taxes as both a public health tool and a sustainable source of domestic health financing.

Through the Kaduna AOP Analytics Platform (KAAP), stakeholders came together to review priorities, analyze performance, and strengthen health sector planning through data, collaboration, and shared accountability.

The engagement provided an opportunity to align planning with sector priorities, identify areas for improvement, and support more informed decision-making for better health outcomes.

HSDF Showcases Evidence-Driven Innovations at APHCF 2026

The Health Strategy and Delivery Foundation (HSDF) participated in the 2nd International Africa Primary Health Care Forum (APHCF 2026), contributing to key discussions on strengthening primary health care systems across Africa through evidence, innovation, and sustainable financing.

HSDF's Ibrahim Ibrahim presented findings from a financial management assessment conducted across 23 PHC facilities, highlighting critical gaps in financial visibility, accountability, claims management, and revenue tracking. The presentation emphasized that while health insurance can improve PHC financing, facilities require stronger financial management systems to effectively manage and account for resources.

Priye Ruth Igali shared insights from HSDF's community-based hypertension and diabetes screening initiative, demonstrating how NCD screening data can inform PHC planning and financing. The programme reached over 2.1 million people, screened 126,100 individuals, and referred more than 27,000 people for further care across four states.

Halima Shehu-Sharif presented how the State of Nutrition Reports consolidate multiple national datasets into state-specific evidence, supporting evidence-informed nutrition planning, priority setting, and equitable resource allocation within Nigeriaโ€™s Primary Health Care system.

In addition, Abolade Oyelere led HSDF's panel session, "Beyond the Blueprint: Driving Frontline PHC Transformation Through Technology, Policy, Advocacy, and Systems Ex*****on," bringing together experts to explore practical pathways for accelerating frontline PHC transformation across Africa.

Through these engagements, HSDF reinforced the importance of data-driven decision-making, stronger health systems, and collaborative action in advancing resilient and sustainable primary health care.

Good health financing starts with good data.

Every decision on health spending should be backed by evidence, not assumptions. To support this, HSDF is providing technical assistance to strengthen the institutionalization of Health Accounts within Kaduna State's health sector.

Using the System of Health Accounts (SHA 2011), stakeholders are able to track how health resources are mobilized, spent, and distributed across government, households, health insurance, development partners, NGOs, and the private sector. This provides the evidence needed for better planning, budgeting, accountability, and resource allocation.

As part of efforts to advance the 2023โ€“2025 Health Accounts exercise, stakeholders completed a five-day training to strengthen their capacity to collect, analyse, and report standardized health expenditure data using the Health Accounts Production Tool (HAPT).

By supporting routine Health Accounts production, HSDF is helping to strengthen evidence-based health financing decisions and build a more responsive health system in Kaduna State.
