𝗙𝗿𝗼𝗺 𝗣𝗶𝗹𝗼𝘁 𝘁𝗼 𝗟𝗼𝗰𝗮𝗹 𝗢𝘄𝗻𝗲𝗿𝘀𝗵𝗶𝗽: 𝗦𝘁𝗿𝗲𝗻𝗴𝘁𝗵𝗲𝗻𝗶𝗻𝗴 𝗖𝗼𝗺𝗺𝘂𝗻𝗶𝘁𝘆 𝗗𝗲𝗮𝘁𝗵 𝗔𝗰𝗰𝗼𝘂𝗻𝘁𝗮𝗯𝗶𝗹𝗶𝘁𝘆 𝗠𝗲𝗰𝗵𝗮𝗻𝗶𝘀𝗺𝘀.

Over the past 33 months, the Health Strategy and Delivery Foundation HSDF, in collaboration with the Lagos State Primary Health Care Board (LSPHCB), piloted the Community-Based Maternal and Perinatal Death Surveillance and Response (CMPDSR) initiative in Ifako-Ijaiye Local Government Area. Stakeholder engagement, advocacy, and technical workshops led to the validation and adoption of the implementation model across the state.

CMPDSR committees were inaugurated at state, LGA, and ward levels, while 𝟓𝟔 frontline health workers and community representatives were trained as verbal autopsy (VA) team members to support maternal death reporting and reviews.

HSDF facilitated a stakeholder engagement and handover meeting with leadership from Ifako-Ijaiye Local Government Area and Ojokoro Local Council Development Area. The meeting highlighted an overview of implementation progress, findings from verbal autopsies conducted over the last 𝟮𝟬 months, the impact of the launch of a toll free line to strengthen maternal death and reporting within the community and sustainability roadmap. A total of 10 maternal deaths were reported with hemorrhagic complications accounting for 𝟱𝟬% of cases. Most deaths were linked to delayed decision-making and weak referral systems.

The intervention was formally handed over to community and LGA leadership, with HSDF continuing technical support to strengthen sustainable, locally owned accountability systems that reduce maternal deaths, while broader insights from the engagement continue to inform ongoing maternal health efforts.

HSDF recently provided technical support to the Q1 2026 State Oversight Committee (SOC) meeting, convened to review progress on the Basic Health Care Provision Fund (BHCPF) implementation. During the meeting, our team assisted with evidence-based discussions on PHC financing flows, facility performance, and system accountability.

The meeting reviewed monitoring findings from PHCs across 23 LGAs and highlighted strong Q1 performance, including full SOC compliance, high Direct Facility Funding (DFF) coverage, timely reporting, improved quality assessment scores, expansion of DFF to additional facilities, and adoption of the PHC-FMS platform.

Discussions also identified persistent financing challenges, including delays in BHCPF disbursements and the absence of state counterpart funding.

HSDF remains committed to strengthening PHC financing, accountability, and sustainable health system performance through technical assistance and improved financial tracking.

Ex*****on is the bridge between planning and results. But what happens when that bridge has gaps?

Through the Gates Foundation-funded Kaduna State Service Delivery Project (KSSDP), HSDF is partnering with the state government to transform Annual Operational Plans (AOPs) into tangible health outcomes for residents. Our recent assessment asked a hard question: 𝗔𝗿𝗲 𝘁𝗵𝗲𝘀𝗲 𝗽𝗹𝗮𝗻𝘀 𝘁𝗿𝘂𝗹𝘆 𝗮𝗰𝗵𝗶𝗲𝘃𝗶𝗻𝗴 𝘁𝗵𝗲𝗶𝗿 𝗼𝗯𝗷𝗲𝗰𝘁𝗶𝘃𝗲𝘀?

The findings are interesting: 𝗪𝗵𝗶𝗹𝗲 𝘀𝘁𝗿𝗼𝗻𝗴 𝗶𝗻𝘀𝘁𝗶𝘁𝘂𝘁𝗶𝗼𝗻𝗮𝗹 𝗳𝗼𝘂𝗻𝗱𝗮𝘁𝗶𝗼𝗻𝘀 𝗲𝘅𝗶𝘀𝘁, 𝗶𝗺𝗽𝗮𝗰𝘁 𝗶𝘀 𝗼𝗳𝘁𝗲𝗻 𝗵𝗶𝗻𝗱𝗲𝗿𝗲𝗱 𝗯𝘆 𝗴𝗮𝗽𝘀 𝗶𝗻 𝗱𝗲𝗰𝗶𝘀𝗶𝗼𝗻-𝗺𝗮𝗸𝗶𝗻𝗴 𝗮𝘂𝘁𝗵𝗼𝗿𝗶𝘁𝘆, 𝗿𝗼𝗹𝗲 𝗰𝗹𝗮𝗿𝗶𝘁𝘆, 𝗮𝗻𝗱 𝗮𝗰𝗰𝗼𝘂𝗻𝘁𝗮𝗯𝗶𝗹𝗶𝘁𝘆 𝘁𝗿𝗮𝗰𝗸𝗶𝗻𝗴.

The good news? A total structural overhaul isn't necessary. By implementing targeted, practical fixes, the state can close the gap between planning and impact.

Over the past five years, Kaduna State has transformed its health workforce, from limited visibility to a system where data informs planning, recruitment is merit-based, and deployment aligns with community needs. Today, that system supports 4,654 health workers across 23 LGAs, all guided by a deliberate, long-term strategy.

This progress did not happen overnight. The 2025 recruitment of 1,800 new health workers (a 39% expansion) was the result of sustained assessment, careful analysis, and strong political commitment.

Our latest brief tells this story in full, highlighting the strategy behind the progress, key lessons learned, and a practical roadmap that other Nigerian states can adopt.

HSDF continues to support the State Government through the Gates Foundation–funded Kaduna State Service Delivery Project, providing technical assistance aimed at improving state-wide health sector planning, costing and sustainable funding for PHC workforce, equipments and service delivery.

At the recent Wellness Expo, HSDF’s Priye Igali shared perspectives on the multi-sectoral drivers of NCDs.

From health systems and workforce capacity to public policy and enabling environments, addressing NCDs goes far beyond clinical care. It requires intentional coordination across sectors and actors.

Priye Igali underscored partnerships as a critical enabler of impact, noting that translating strategy into tangible outcomes requires:

Transforming Maternal Care: The Project Aisha Success Story.

Nigeria continues to face serious maternal health challenges but in Lagos, Project Aisha is changing the narrative.

By prioritizing data over assumptions, the initiative trained 79 quality improvement (QI) mentors, who have since equipped 158 frontline health workers with critical, life-saving skills.

The impact?
(1) 99% maternal survival rate.
(2) 55% reduction in facility-based maternal deaths.
(3) Standardized care tools + real-time digital feedback powered by NaviHealth.ai

What started in just 4 LGAs has now been scaled across all 20 LGAs and is fully integrated into the Lagos State Health District budget. This ensures high-quality, community-driven maternal care is not just a project, but a systemic practice.

HSDF is proud to be represented at HCA Consult’s Wellness Expo this Wednesday at the Landmark Event Center.

Our colleague, Priye Ruth Igali, will be speaking on a panel themed “Fighting the Silent Killers: Wellness Interventions for Preventing Non-Communicable Diseases,” where she will share insights from HSDF’s work across our NCDI and WDF-supported initiatives.

The session will explore how wellness interventions can be more effectively leveraged to prevent NCDs, the critical role of partnerships in scaling impact, and practical approaches to reaching underserved and lower-income populations.
