Does your child or young person have a health passport? ๐Ÿ“‹

A health passport is a document that helps healthcare professionals understand your child better. It can include information about:

๐Ÿ’šHow they communicate
๐Ÿ’šWhat helps them feel safe
๐Ÿ’š Sensory needs and triggers
๐Ÿ’š Medical conditions and medications
๐Ÿ’š Things they may find difficult
๐Ÿ’š How they express pain or distress

For many families, a health passport can also reduce the need to repeatedly explain their child's needs to different healthcare professionals. Instead of having the same conversations over and over again, important information is available in one place for staff to refer to.

Sharing a health passport before a hospital appointment, admission or emergency visit can help staff provide more personalised care and reduce stress for both your child and your family.

Many NHS trusts provide their own templates, often called a health passport, hospital passport, learning disability passport or communication passport.

If your child/young person doesn't already have one, try searching your local NHS trust's website or asking your GP surgery, learning disability nursing team, paediatrician or hospital for a template.

๐Ÿ’ก Keep a digital copy on your phone so it's always available if you need unexpected medical care.

Has a health passport helped your family? We'd love to hear your experiences in the comments ๐Ÿ‘‡

Can you spare me 5-10 minutes of your time?

Please take a look at the following article, published by We Care You Care, in conjunction with RCBC, Mbro Council and partners.

https://wecareyoucare.info/articles/is-the-new-all-age-carers-strategy-right-tell-us-what-you-think

We have developed a new South Tees Carers Strategy draft and before we publish it, we are seeking to hear the voices of the people it matters to.
For ease, a poster has been developed which highlights the key areas that the strategy aims to address. There is a very short survey that should take around 5 minutes to complete.

Happy International Children's Day!

Today feels like the right day to share some news we're really proud of. With the SEND reforms still leaving us with so much uncertainty, we wanted to do something practical for families in our community.

So from September, our Primary Education Programme (PEP) and Early Years Development Programme (EYDP) are completely free for all educators.

That means any teacher, TA, SENCo or early years practitioner can now access expert Down syndrome training, practical resources and ongoing support - at no cost to them or their setting.

Our programmes are designed to support teaching in every way possible. They're easy to use, they can be dipped in and out of at any time. We're very excited to make these free and hopefully with that, reach more settings across the country.

Send this to your educators to pre-register now and we'll invite them to our new platform when it launches in September. https://secure.downsyndromeuk.co.uk/sign-up

Or refer them and we'll invite them: https://forms.office.com/e/9b3H0BUFKn
