Attached you can find a copy of the rules, affidavit, team roster sheet, and sponsor sheets. Sponsorships can be completed online as well.
If you register online, please be sure to notify your team captain so they can include you on the team roster and provide your team captain with your signed affidavit (a copy is attached for your convenience). AFFIDAVIT MUST BE PRINTED AND SIGNED AND HANDED IN.
