The Community Connections Center’s partner agencies provide a variety of services and programs to the Mount Prospect community. The principal partner agencies are the Village of Mount Prospect Human Services and Police Departments and the Mount Prospect Public Library. Additional partner agencies include Community Consolidated School District 59 and WINGS. This FACEBOOK page represents the Village
of Mount Prospect Human Services Department at its satellite site in the Community Connections Center. Our Partner Agencies have their own FACEBOOK pages offering additional information. Social Services include: 1) Assistance filling out local, State and Federal assistance applications (examples: area hospital assistance, Illinois Medicaid and Supplemental Nutrition Assistance Program, Illinois Benefits Access programs, Medicare D, etc.). 2) Financial assessments to qualify for the Village’s food pantry, emergency assistance, summer family pool pass, school supplies, holiday meals. 3) Information and referrals. 4) Counseling through police and school based referrals. 5) Crisis intervention and victim court advocacy. Nursing and Health Services include: 1) Adult blood pressure, cholesterol and diabetes screenings. 2) Diabetes Education and Support Group. 3) Health education seminars. 4) Medical equipment lending closet (coordinated with the Human Services main office). Programming includes: 1) Play and Learn Program: Parents and their 2 ½ through 5 year old children enjoy arts, crafts, exercise, stories and music. 2) My School. My Choice.: Holmes Jr. High School students participate in a summer book club and enjoy field trips. Parent selected discussions supporting positive parent-child conversations. 3) Conversations in English: Conversational English for adults on a variety of topics offered for beginning and intermediate English-speaking levels. 4) Ask Your Village: Community Development, Police and Human Services Departments meet to answer residents’ questions and address concerns. 5) Welcome to the Village: New residents are welcome to tour and learn more about the Center and Village services. Police Services include: infant-child safety seat checks and responding to questions on law enforcement matters.
